Indications

Acifix is indicated for the treatment of:

  • Active duodenal and benign gastric ulcers: To promote healing.
  • Erosive or ulcerative Gastro-esophageal Reflux Disease (GERD): For both short-term treatment and long-term management.
  • Symptomatic GERD: For moderate to very severe symptoms.
  • Zollinger-Ellison Syndrome: A condition involving excessive stomach acid.
  • H. pylori eradication: Used in combination with other antibiotics to treat Helicobacter pylori infection in patients with peptic ulcer disease.

 

Pharmacology

Rabeprazole, the active ingredient in Acifix, is a proton pump inhibitor. It suppresses stomach acid secretion by targeting and inhibiting the gastric H+/K+-ATPase enzyme, which is the acid (proton) pump in the stomach's parietal cells.


 

Dosage

The recommended dosage varies depending on the condition:

  • Active Duodenal or Benign Gastric Ulcer: 20 mg once daily in the morning. Treatment for duodenal ulcers typically lasts four weeks, while gastric ulcers require six weeks, though some patients may need additional treatment time.
  • GERD: 20 mg once daily for four to eight weeks for erosive or ulcerative GERD. For long-term management, a maintenance dose of 10 mg or 20 mg once daily is used. For symptomatic GERD, 10 mg once daily is recommended.
  • Pediatric GERD (1 to 11 years): Doses are 5 mg once daily for children under 15 kg and 10 mg once daily for children 15 kg or more, for up to 12 weeks.
  • Zollinger-Ellison Syndrome: The starting dose is 60 mg once daily, which may be increased up to 120 mg/day. Doses over 100 mg may require a twice-daily regimen.
  • H. pylori Eradication: A 7-day course of 20 mg Acifix twice daily, along with clarithromycin 500 mg twice daily and amoxicillin 1g twice daily.

 

Administration

For once-daily treatment, tablets should be taken in the morning before eating. The tablets should be swallowed whole and not chewed or crushed. This regimen helps with compliance.


 

Interaction

Acifix can significantly inhibit gastric acid secretion, which may affect the absorption of other drugs that depend on stomach pH. Co-administration with ketoconazole or itraconazole may reduce their effectiveness. It is not recommended to take Acifix with atazanavir. No interaction with liquid antacids has been observed.


 

Contraindications

This medication is contraindicated in patients who are hypersensitive to rabeprazole or any of its ingredients. It is also contraindicated during pregnancy and breastfeeding.


 

Side Effects

Acifix is generally well-tolerated. Side effects may include headache, diarrhea, abdominal pain, vomiting, constipation, dry mouth, changes in appetite, muscle pain, drowsiness, and dizziness.


 

Pregnancy & Lactation

Acifix is in US FDA pregnancy category 'C'. Animal studies have not shown harm to the fetus, but there are no adequate studies in pregnant women. Since rabeprazole is likely excreted in human milk, a decision must be made to either stop nursing or stop the drug, based on its importance to the mother.


 

Precautions & Warnings

  • Before starting treatment, it's important to rule out the presence of a gastric or esophageal malignancy.
  • Patients on long-term treatment should be regularly monitored.
  • Proton pump inhibitors may slightly increase the risk of fractures, especially in the elderly. Patients at risk for osteoporosis should ensure adequate vitamin D and calcium intake.
  • There have been rare post-marketing reports of blood disorders and liver enzyme abnormalities, which typically resolve upon discontinuing the drug.
  • Caution should be exercised when initiating treatment in patients with severe liver dysfunction.
  • Treatment with proton pump inhibitors may increase the risk of gastrointestinal infections.
  • Prolonged use may lead to low magnesium levels (hypomagnesaemia) and may reduce vitamin B12 absorption.
  • Proton pump inhibitors are associated with a very infrequent skin condition called Subacute Cutaneous Lupus Erythematosus (SCLE).
  • It may interfere with laboratory tests for neuroendocrine tumors; treatment should be stopped for at least 5 days before such tests.

 

Use in Special Populations

  • Renal and hepatic impairment: No dosage adjustment is necessary for patients with kidney or liver impairment.
  • Pediatric populations: Acifix is not recommended for children due to a lack of safety and efficacy data.

 

Overdose Effects

The effects of an overdose are generally minimal and reversible. There is no specific antidote. Treatment should be symptomatic and supportive.
