Agotin

Generic Name: Agomelatine

Indications:

• Major depressive disorder in adults, especially in patients not responding to or intolerant of selective serotonin reuptake inhibitors (SSRIs)

• Prevention of relapse in major depressive disorder

Pharmacology:

• Agomelatine is a melatonergic agonist (MT1 and MT2 receptors) and a 5-HT2C antagonist.

• It helps to resynchronize circadian rhythms.

• It increases noradrenaline and dopamine release specifically in the frontal cortex.

Dosage & Administration:

• The effective dose is 25 mg per day, taken once at bedtime.

• The dose may be increased to 50 mg per day in patients with an inadequate response after two weeks.

• It can be taken with or without food.

Contraindications:

• Hypersensitivity to the active substance or any of the excipients

• Hepatic impairment (i.e., cirrhosis or active liver disease) or transaminases exceeding three times the upper limit of normal

• Concomitant use of potent CYP1A2 inhibitors (e.g., fluvoxamine, ciprofloxacin)

Side Effects:

Commonly reported side effects include:

• Headache

• Nausea

• Diarrhea

• Increased liver enzyme levels

Precautions:

• Liver function should be monitored before starting therapy and periodically during treatment.
