Indications
This medication treats essential hypertension, angina pectoris with coexisting hypertension, post-myocardial infarction management, and refractory angina unresponsive to nitrate therapy.

Pharmacology
Combines amlodipine (dihydropyridine calcium channel blocker) and atenolol (cardioselective β-blocker). Amlodipine reduces peripheral vascular resistance by inhibiting calcium influx in vascular smooth muscle. Atenolol lowers blood pressure via reduced cardiac output, suppressed renin activity, and decreased myocardial oxygen demand through negative chronotropic/inotropic effects.

Dosage & Administration

• Standard dose: 1 tablet (amlodipine 5 mg/atenolol 25 mg) once daily

• Titration: May increase to 2 tablets daily if needed under medical supervision
  Individualize dosing based on therapeutic response.

Interaction

• Disopyramide: Reduced clearance (↑ toxicity risk)

• Ampicillin (≥1g): May lower atenolol levels

• Antidiabetics: Potential reduced insulin sensitivity (atenolol)

Contraindications
Hypersensitivity to components, sinus bradycardia, 2nd/3rd-degree heart block, cardiogenic shock, hypotension, congestive heart failure, severe left ventricular dysfunction.

Side Effects
Generally well-tolerated. May include fatigue, headache, peripheral edema, nausea, drowsiness, anxiety, or depression.

Pregnancy & Lactation
Use only if benefits outweigh fetal risks. Discontinue breastfeeding during treatment.

Precautions & Warnings

• Asthma/COPD: Use cautiously (atenolol may cause bronchospasm)

• Renal impairment: Monitor if CrCl <30 ml/min (↓ atenolol excretion)

• Hepatic impairment: Caution in severe liver disease (↑ amlodipine half-life)

• Withdrawal: Taper gradually to avoid rebound ischemia

• Coronary risk: Do not abruptly discontinue in CAD patients

Overdose Effects
Hypotension, acute heart failure. Manage with gastric lavage/activated charcoal and symptomatic support.
