Aminosol Gold IV 10%

Presentation: Sterile, aqueous solution containing 10% amino acids (essential and non-essential) and electrolytes. Available in 500 ml bottles for intravenous infusion. Manufactured by OSL Pharma Limited and possibly other companies in Bangladesh.

Indications: Indicated as a source of amino acids and electrolytes in adult and pediatric patients needing IV nutrition, particularly those with basal amino acid requirements. May be suitable for patients with moderate to severe metabolic stress.

Dosage and Administration: Dosage is individualized based on the patient's condition, nutritional status, body weight, and metabolic needs. The nitrogen requirement for maintenance typically ranges from 0.10-0.25 g nitrogen/kg/day, corresponding to 11-27 ml/kg/day of this 10% solution. The infusion rate should be slow and controlled, not exceeding 1000 ml in 6 hours (approximately 2.8 ml/min). In obese patients, the dose should be based on ideal body weight. In infants and children, a maximal rate of 30 ml/kg/day is recommended with a gradual increase during the first week. Administer intravenously under strict medical supervision.

Side Effects: Generally well-tolerated. Nausea occurs rarely. Vomiting, flushing, and sweating have been observed with infusion rates exceeding the recommended maximum. Transient increases in liver function tests and hypersensitivity reactions have been reported. Thrombophlebitis may occur with peripheral vein infusion. Hyperphenylalaninemia may occur in severely ill, premature infants.

Precautions: Contraindicated in patients with inborn errors of amino acid metabolism, severe liver damage, and severe uremia without dialysis. Due to the glucose content (often present in formulations with amino acids and electrolytes), it may be contraindicated in hyperosmolar nonketotic diabetic coma. Use with caution in patients with diabetes mellitus, severe heart failure, or renal impairment with fluid restriction or oliguria/anuria. Monitor fluid balance, electrolyte levels, and blood glucose regularly. Ensure adequate phosphate supply in hypophosphatemia. Do not use if the solution is turbid or contains particles.

Storage: Store at controlled room temperature, protected from light and freezing. Keep out of reach of children. Discard any unused portion.
