Amisol IV 

Presentation: Amisol IV, available in Bangladesh, is an intravenous infusion containing a sterile aqueous solution of crystalline Amino Acids and D-Sorbitol with Electrolytes. It is available in 500 ml bottles. Different formulations such as Amisol Gold with higher concentrations of amino acids and added glucose are also available.

Indications: Amisol IV is indicated as a source of amino acids for protein synthesis in patients requiring intravenous nutrition, particularly those with basal amino acid requirements. It is also used to aid faster recovery in conditions like post-surgery, burns, renal insufficiency, hepatic insufficiency, and in the management of cancer. The formulation with glucose (e.g., Amisol Gold) also provides a source of carbohydrates and electrolytes.

Dosage and Administration: The dosage is determined by the patient's nutritional status and the degree of metabolic stress, typically ranging from 0.10-0.25 g nitrogen/kg/day, which corresponds to 11-35 ml of the infusion/kg/day. The infusion rate should be slow, not exceeding 1000 ml in 6 hours (approximately 2.8 ml/min). In children and infants, a maximal rate of 30 ml/kg body weight/day is recommended with a gradual increase during the first week. It is administered intravenously under medical supervision.

Side Effects: Amisol IV is generally well-tolerated. Nausea occurs rarely. Rapid infusion or large quantities may lead to nausea, vomiting, and sweating. Thrombophlebitis may occur with peripheral vein use. Osmotic diuresis and dehydration may result from exceeding dosage recommendations. Hyperglycemia is also a potential risk with glucose-containing formulations. Increased urinary excretion of trace elements like copper and zinc may occur during prolonged use.

Precautions: Amisol IV is contraindicated in patients with inborn errors of amino acid metabolism, severe liver damage, severe uremia without dialysis facilities, and hyperosmolar nonketotic diabetic coma (for glucose-containing solutions). Caution should be exercised in patients with diabetes mellitus, severe heart failure, or renal impairment with fluid restrictions. Monitoring of serum potassium and blood glucose is recommended during rapid or high-volume infusion.

Use in Pregnancy and Breastfeeding: The safety of Amisol IV during pregnancy and breastfeeding has not been fully established. It should be used only if clearly needed and under medical supervision.  

Storage: Store protected from light at temperatures below 25°C. Avoid freezing. Keep out of reach of children.
