Presentation: Aproxi 80 Capsule, available in Bangladesh, contains Aprepitant 80 mg.

Indications: Aproxi (Aprepitant) is indicated for:

  • Prevention of acute and delayed nausea and vomiting associated with highly emetogenic cancer chemotherapy. This includes chemotherapy regimens containing cisplatin.
  • Prevention of nausea and vomiting associated with moderately emetogenic cancer chemotherapy.
  • Prevention of postoperative nausea and vomiting (PONV) in adults.

Dosage and Administration: Aproxi capsules are for oral administration and can be taken with or without food. The dosage regimen depends on the indication and the specific chemotherapy regimen. Always follow your doctor's precise instructions.

For Chemotherapy-Induced Nausea and Vomiting (CINV):

A common regimen for highly emetogenic chemotherapy includes:

  • Day 1: Aprepitant 125 mg orally 1 hour prior to chemotherapy.
  • Day 2 and Day 3: Aprepitant 80 mg orally once daily in the morning.

For moderately emetogenic chemotherapy, similar regimens may be used, sometimes with a shorter duration.

For Postoperative Nausea and Vomiting (PONV):

  • The recommended oral dosage is 40 mg within 3 hours prior to induction of anesthesia.

Side Effects: Common side effects reported with Aproxi (Aprepitant) may include:

  • Headache
  • Fatigue
  • Loss of appetite
  • Constipation
  • Diarrhea
  • Nausea
  • Hiccups

Less common side effects can include dizziness, abdominal pain, and changes in liver function tests. Allergic reactions, although rare, can occur.

Precautions: Aproxi (Aprepitant) is contraindicated in patients with known hypersensitivity to aprepitant or any component of the product. It should not be used concurrently with pimozide, terfenadine, astemizole, or cisapride due to potential serious interactions.

Caution should be exercised in patients with liver disease, as aprepitant is metabolized by the liver. Aprepitant can interact with several other medications, including corticosteroids (like dexamethasone, the dosage of which may need adjustment), warfarin, and hormonal contraceptives (making them less effective; alternative contraception should be used during and for 1 month after treatment).

Inform your doctor about all medications you are currently taking before starting Aproxi. The safety and efficacy of Aprepitant in patients under 18 years of age have not been fully established.

Use in Pregnancy and Breastfeeding: Aproxi should be used during pregnancy only if clearly needed and the potential benefit outweighs the risk to the fetus. It is not known whether aprepitant is excreted in human milk; therefore, caution should be exercised when administering Aproxi to a breastfeeding woman. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Storage: Store below 30°C (86°F). Keep out of reach of children. Protect from moisture.
