Indications: Aptin M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus whentreatment with both vildagliptin andmetformin is appropriate. This combination is used in patients whose blood sugar is not adequately controlled with metformin or vildagliptin alone, or in patients already treated with the combination of vildagliptin and metformin as separate tablets.
Dosage and Administration: The dosage of Aptin M should be individualized based on the patient's current regimen, effectiveness, and tolerability, while not exceeding the maximum recommended daily dose ofvildagliptin 100 mg and metformin 2000 mg. It is usually taken twice daily, once in the morning and once in the evening, with or just after food to reduce gastrointestinal side effects associated with metformin.Patients already treated with vildagliptin and metformin as separate tablets may be switched to Aptin M containing the same doses of each component.Tablets should be swallowed whole and not crushed, split, or chewed.Renal function should be assessed before initiating Aptin M and periodically thereafter.
Side Effects:Common side effects may include nausea, vomiting, diarrhea, stomach pain, loss of appetite, headache, and dizziness.Hypoglycemia (low blood sugar) can occur, especially when Aptin M is used in combination with other antidiabetic agents like insulin or sulfonylureas.Other possible side effects include sore throat, weakness, and metallic taste.
Precautions:Aptin M is contraindicated in patients with severe renal impairment, acute or chronic metabolic acidosis (including diabetic ketoacidosis),and known hypersensitivity tovildagliptin or metformin.It should be used with caution in patients with moderate renal impairment, liver disease, and a history of pancreatitis. Excessive alcohol intake should be avoided due to the increased risk of lactic acidosis associated with metformin.Aptin M is generally not recommended during pregnancy or breastfeeding.
