Indications

Bactin 0.3% ophthalmic solution is indicated for the treatment of:

  • Corneal ulcers
  • Conjunctivitis
  • Blepharitis These conditions must be caused by susceptible strains of bacteria.

 

Composition

Each ml of eye drops contains 3 mg of Ciprofloxacin (as Ciprofloxacin Hydrochloride BP) and 0.06 mg of Benzalkonium Chloride as a preservative.


 

Pharmacology

Ciprofloxacin is a quinolone antibiotic with broad-spectrum activity against both Gram-negative and Gram-positive bacteria. It works by inhibiting the A subunits of DNA gyrase, an enzyme essential for bacterial DNA replication, thereby acting as a bactericidal agent.


 

Dosage & Administration

  • For Corneal Ulcers:
    • Day 1: Instill 2 drops into the affected eye every 15 minutes for the first 6 hours, then every 30 minutes for the rest of the day.
    • Day 2: 2 drops every hour.
    • Day 3-14: 2 drops every 4 hours. Treatment may continue if the cornea is not fully healed.
  • For Bacterial Conjunctivitis/Blepharitis: Instill 1 drop every 2 hours for 2 days, followed by 1 drop every 4 hours for the next 5 days.
  • Pediatric Use: Safety and effectiveness in children under 1 year of age have not been established.

 

Interaction

While specific studies for the ophthalmic solution have not been done, systemic quinolones can interact with:

  • Theophylline: Elevates plasma concentrations.
  • Caffeine: Interferes with metabolism.
  • Warfarin: Enhances its anticoagulant effects.
  • Cyclosporin: May cause transient elevations in serum creatinine.

 

Contraindications

Bactin is contraindicated in patients with a history of hypersensitivity to Ciprofloxacin, any other component of the product, or other quinolones (such as Nalidixic acid).


 

Side Effects

The most common side effect is local burning or discomfort. Other reported reactions include lid margin crusting, foreign body sensation, itching, conjunctival hyperemia, and a bad taste after instillation. In corneal ulcer studies, white crystalline precipitates were observed in about 17% of patients. Less than 1% of patients reported corneal staining, allergic reactions, lid edema, or decreased vision.


 

Pregnancy & Lactation

There are no adequate studies on pregnant women. Bactin should only be used during pregnancy if the potential benefit outweighs the potential risk to the fetus.


 

Precautions & Warnings

  • Prolonged use may lead to the overgrowth of non-susceptible organisms, including fungi. If a super-infection occurs, discontinue use and seek alternative therapy.
  • Discontinue Bactin at the first sign of a skin rash or other hypersensitivity reaction.

 

Overdose Effects

In case of a topical overdose, the eye(s) can be flushed with warm tap water.


 

Therapeutic Class

Bactin is an Ophthalmic antibacterial drug.


 

Storage Conditions

Store at room temperature and protect from light. It is recommended not to use the contents more than one month after the bottle is first opened.
