Betacor (Bisoprolol Fumarate): A Selective Beta-Blocker for Cardiovascular Conditions

Betacor contains Bisoprolol Fumarate, a highly selective beta-1 (β1​) blocker. It is indicated for various cardiovascular conditions, primarily by reducing heart rate and cardiac output. It belongs to the therapeutic class of antiadrenergic agents (beta-blockers) and beta-adrenoceptor blocking drugs.

How Betacor Works (Pharmacology)

Bisoprolol Fumarate is the most selective β1​ blocker available, demonstrating the highest affinity for the β1​ receptor. It works by:

• Selectively blocking $\beta$$1$​ adrenergic receptors: These receptors are found predominantly in the heart and vascular smooth muscle.
• Reducing heart rate and cardiac output: By blocking these receptors, Bisoprolol slows the heart rate and decreases the amount of blood pumped by the heart, leading to a reduction in arterial hypertension (blood pressure).
• Minimal impact on lipid metabolism: Unlike some non-β1​ selective beta-blockers, Bisoprolol does not cause adverse changes in cholesterol fractions, including the cardioprotective HDL-cholesterol, during long-term therapy.

Pharmacokinetic Properties:

• Absorption and Bioavailability: Almost completely absorbed ($>90<$10%). Food intake does not affect absorption.
• Distribution: Extensively distributed with a medium distribution volume of 3.5 L/kg.
• Metabolism: Primarily metabolized via oxidative pathways, mainly by CYP3A4 (∼95%) with a minor role by CYP2D6. All metabolites are very polar and renally eliminated, without pharmacological activity.
• Elimination: Clearance is balanced between renal elimination of the unchanged molecule (∼50%) and hepatic metabolism (∼50%). The total clearance is approximately 15 L/h, and it has an elimination half-life of 10-12 hours, supporting once-daily dosing.

Key Indications & Benefits

Betacor is indicated for:

• Hypertension (high blood pressure)
• Angina (chest pain due to reduced blood flow to the heart)
• Moderate to severe heart failure

Important Note: Betacor is not recommended for the emergency treatment of hypertensive crises.

Dosage & Administration

The dosage of Bisoprolol Fumarate must be individualized to the patient's needs. Always consult a registered physician for medication use.

• Adults (Mild to Moderate Hypertension):
  • Usual starting dose: 5 mg once daily, either alone or added to a diuretic.
  • If response is inadequate, the dose may be increased to 10 mg, and then if necessary, to 20 mg once daily.
  • An appropriate interval for dose titration is 2 weeks.
  • Increasing the dose beyond 20 mg once daily provides only a small additional benefit.
• Children: Safety and effectiveness in children have not been established.
• Patients with Renal or Hepatic Impairment (hepatic impairment like hepatitis or cirrhosis; renal dysfunction with creatinine clearance less than 40 mL/min):
  • Initial daily dose should be 5 mg.
  • Caution must be used in dose titration due to the possibility of accumulation.
  • Bisoprolol fumarate is not dialyzable, so drug replacement is not necessary in patients undergoing dialysis.
• Geriatrics (Elderly):
  • Usually, no dose adjustment is necessary, unless there is also significant renal or hepatic dysfunction.

Contraindications:

• Cardiogenic shock
• Overt heart failure
• Second or third-degree A-V block (a type of heart block)
• Right ventricular failure secondary to pulmonary hypertension
• Sinus bradycardia (abnormally slow heart rate)

Side Effects:

Side effects are usually mild and transient.

• Common: Fatigue, dizziness, headache, gastrointestinal disturbances (e.g., nausea, vomiting, diarrhea, constipation, abdominal pain).
• Other: Cold or numb extremities (hands and feet), muscle weakness or cramps, slower than normal heart rate, breathing difficulties due to bronchospasm (narrowing of airways) in people with asthma or COPD.

Pregnancy & Lactation:

• Pregnancy: Animal studies showed fetotoxic and embryolethal effects at higher doses but were not teratogenic. There are no studies in pregnant women. Bisoprolol fumarate should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
• Lactation: Small amounts of bisoprolol fumarate have been detected in the milk of lactating rats. It is not known whether this drug is excreted in human milk. If use of bisoprolol fumarate is considered essential, mothers should stop nursing.

Precautions & Warnings:

• Impaired Renal or Hepatic Function: Use caution when adjusting the dose in these patients.
• Severe Anaphylactic Reactions: Patients with a history of severe anaphylactic reactions to various allergens may be more reactive and unresponsive to usual doses of epinephrine while taking beta-blockers.

Drug Interactions:

• Other $\beta$-blocking Agents: Betacor should not be combined with other β-blocking agents.
• Catecholamine-Depleting Drugs (e.g., reserpine, guanethidine): Monitor closely for excessive reduction of sympathetic activity (e.g., marked bradycardia, hypotension).
• Centrally Active Antihypertensive Agents (e.g., clonidine): β-blockers may worsen rebound hypertension upon clonidine withdrawal. If co-administering, the β-blocker should be withdrawn several days before clonidine. If replacing clonidine with a β-blocker, delay introduction of the β-blocker for several days after clonidine cessation.
• Antiarrhythmic Agents (e.g., disopyramide) or certain Calcium Antagonists (e.g., verapamil, diltiazem): Use with care. Combined use with myocardial depressants or inhibitors of A-V conduction can lead to prolongation of S-A and A-V conduction, particularly in patients with impaired ventricular function or conduction abnormalities. This may result in severe hypotension, bradycardia, and cardiac failure.
