Indications

This topical preparation is indicated for the treatment of inflammatory dermal infections, specifically:

  • Tinea pedis (athlete's foot)
  • Tinea cruris (jock itch)
  • Tinea corporis (ringworm)

 

Pharmacology

This preparation contains two active ingredients:

  • Clotrimazole: A broad-spectrum antifungal agent that works by inhibiting the synthesis of ergosterol, a major component of the fungal cell membrane. This leads to cell membrane instability and the eventual death of the fungus.
  • Betamethasone dipropionate: A corticosteroid with anti-inflammatory, anti-pruritic (anti-itching), and vasoconstrictive (blood vessel narrowing) properties. Its exact mechanism of action is not fully known.

 

Dosage & Administration

  • Apply a sufficient amount to the affected and surrounding skin areas twice daily (morning and evening).
  • The recommended duration of use is 2 weeks for tinea cruris and tinea corporis, and 4 weeks for tinea pedis.
  • The use of this cream for longer than four weeks is not recommended.
  • The safety and effectiveness of this preparation have not been established in children under 12 years of age.

 

Contraindications

This topical preparation is contraindicated in patients with:

  • Known sensitivity to any of its components, other corticosteroids, or imidazoles.
  • Facial rosacea, acne vulgaris, perioral dermatitis, perianal and genital pruritus, or napkin (diaper) eruptions.
  • Bacterial or viral infections. If irritation or sensitization occurs, discontinue treatment.

 

Side Effects

Reported adverse reactions include:

  • Common: Burning and dry skin, paresthesia (abnormal sensations like tingling).
  • Less Common: Rash, edema, and secondary infection.

 

Pregnancy & Lactation

  • Pregnancy: There is insufficient evidence of safety during pregnancy. It should be used only if the potential benefit justifies the risk to the fetus, and use should not be extensive (large amounts or for long periods).
  • Lactation: It is unknown if the components are excreted in human milk, so caution should be exercised when treating nursing mothers.

 

Precautions & Warnings

  • Systemic absorption of topical corticosteroids can cause reversible suppression of the hypothalamic-pituitary-adrenal (HPA) axis. If this occurs, attempt to withdraw the drug or reduce the frequency of application.
  • Pediatric patients are more susceptible to systemic toxicity from topical corticosteroids due to a larger skin surface-to-body mass ratio.

 

Overdose Effects

Acute overdose is unlikely to be life-threatening. The cream should not be used for longer than the prescribed duration.
