Bigmet 500     

Presentation:Bigmet 500 tablets contain 500 mg of metformin hydrochloride.They are typically film-coated and are available in blister packs or strips.  

Indications: Bigmet 500 is indicated for:  

  • The treatment of type 2 diabetes mellitus, particularly in overweight patients, when diet and exercise aloneare not sufficient to achieve adequate glycemic control.  
  • It can be used as monotherapy or in combination with other oral antidiabetic agents or with insulin.  
  • In children from 10 years of age and adolescents,it can be used as monotherapy or in combination with insulin.  
  • Metformin is also used in the treatment of polycystic ovary syndrome (PCOS) in women, helping to improve insulin sensitivity and regulate hormone levels.  

Dosage and Administration:The dosage of Bigmet 500 should be individualized based on blood glucose levels and tolerability.  

  • Adults:The usual starting dose is 500 mg twice a day or 850 mg once a day, taken with meals.The dosage may be gradually increased as needed, up to a maximum daily dose of 2000 mg or 2550 mg in divided doses.  
  • Children (10 years and older) and Adolescents:The usual starting dose is 500 mg once or twice a day, given with meals.The dosage may be gradually increased up to a maximum daily dose of 2000 mg in divided doses.Bigmet 500 tablets should be swallowed whole and should be taken with food to help reduce gastrointestinal side effects.It is important to follow your doctor's instructions regarding dosage and timing.

Side Effects: Common side effects of Bigmet (metformin) may include:

  • Nausea  
  • Vomiting  
  • Diarrhea  
  • Abdominal pain  
  • Loss of appetite  
  • Taste disturbance (metallic taste)  

These side effects are often mild and temporary, occurring more frequently when starting treatment and usually subsiding as the body adjusts.  

Less common but more serious side effects can include lactic acidosis, a rare but potentially life-threatening condition.Symptoms of lactic acidosis may include deep and rapid breathing, persistent nausea, vomiting, and stomach pain.If you experience these symptoms, seek immediate medical attention.

Precautions: Bigmet 500 is contraindicated in patients with:

  • Severe renal impairment (eGFR < 30 mL/min/1.73 m²).
  • Acute metabolic acidosis, including diabetic ketoacidosis.
  • Known hypersensitivity to metformin.  
  • Acute conditions that may alter kidney function, such as dehydration, severe infection, and shock.
  • Acute or chronic disease that may cause tissue hypoxia, such as cardiac or respiratory failure.  
  • Hepatic impairment.  
  • Acute alcohol intoxication or chronic alcoholism.

Caution should be exercised in patients with moderate renal impairment (eGFR between 30 and 45 mL/min/1.73 m²), elderly patients, and those taking certain medications that may affect kidney function or increase the risk of lactic acidosis. Alcohol consumption should be avoided as it can increase the risk of side effects, including hypoglycemia and lactic acidosis.  

Storage:Store Bigmet 500 tablets in a cool and dry place, protected from light and moisture.Keep out of reach of children.
