Presentation:Each tablet of Bigmet 850 contains 850 mg of metformin hydrochloride.Bigmet 850 is available in blister packs containing a specific number of tablets.Please refer to the product packaging for exact details. Bigmet is a brand of Renata Limited in Bangladesh.
Indications: Bigmet (metformin hydrochloride) is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight patients, when dietary management and exercise alone do not result in adequate glycemic control.
Dosage and Administration: The usual starting dose of Metformin is 500 mg twice a day or 850 mg once a day, given with meals.10 Dosage increases should be made in increments of 500 mg weekly or 850 mg every 2 weeks, up to a total of 2000 mg per day, given in divided doses.11 For those patients requiring additional glycemic control,12 the dose may be increased up to a maximum daily dose of 2550 mg, usually given three times a day with meals.
The dosage should be individualized based on effectiveness and tolerance, while not exceeding the maximum recommended daily dose.
Side Effects:Common side effects may include nausea, vomiting, diarrhea, abdominal pain, and loss of appetite.These are often transient and occur at the beginning of treatment. A metallic taste may also occur. Long-term use may be associated with vitamin B12 deficiency.Lactic acidosis is a rare but serious potential side effect.
Precautions: Bigmet is contraindicated in patients with hypersensitivity to metformin, acute metabolic acidosis, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute conditions with the potential to affect renal function (such as dehydration, severe infection, shock), acute or chronic disease which can cause tissue hypoxia (such as cardiac or respiratory failure, recent myocardial infarction), hepatic insufficiency, acute alcohol intoxication,and during breastfeeding. Caution should be exercised in patients with moderate renal impairment (eGFR between 30 and 45 mL/min/1.73 m²), and treatment should be reviewed. Alcohol consumption should be avoided. Temporary discontinuation of metformin is usually advised before surgical procedures or iodinated contrast studies.
