Bizoran 5/20: Dual-Action for Effective Blood Pressure Control

Bizoran 5/20 is a powerful prescription medication indicated for the treatment of hypertension (high blood pressure), either alone or in combination with other antihypertensive agents. It's particularly useful as an initial therapy for patients who are likely to need multiple medications to reach their blood pressure goals. The decision to start with a combination therapy like Bizoran should be carefully individualized, considering factors like baseline blood pressure, target goals, and the increased likelihood of achieving those goals compared to using a single medication.


How Bizoran 5/20 Works (Pharmacology)

Bizoran 5/20 combines two effective blood pressure-lowering agents: Amlodipine and Olmesartan Medoxomil, each working through different mechanisms to provide comprehensive control.

  • Amlodipine (Calcium Channel Blocker): Amlodipine is a dihydropyridine calcium channel blocker. It works by inhibiting the influx of calcium ions into both vascular smooth muscle and cardiac muscle cells. Its primary action is a potent peripheral arterial vasodilator, directly relaxing the smooth muscles in blood vessel walls. This leads to a reduction in peripheral vascular resistance and, consequently, a decrease in blood pressure.
  • Olmesartan Medoxomil (Angiotensin II Receptor Blocker - ARB): Angiotensin II is a powerful hormone that causes blood vessels to constrict and stimulates aldosterone secretion, both of which contribute to high blood pressure. Olmesartan Medoxomil selectively blocks the binding of Angiotensin II to the AT1 receptor, found in various tissues like vascular smooth muscle and adrenal glands. By blocking these receptors, Olmesartan Medoxomil prevents Angiotensin II's vasoconstricting and aldosterone-secreting effects, thereby lowering blood pressure. Importantly, Olmesartan Medoxomil does not inhibit ACE (Angiotensin-Converting Enzyme).

This dual mechanism provides a robust approach to lowering blood pressure.


Dosage & Administration

The dosage of Bizoran should be carefully determined by your physician, particularly when substituting for individual components.

  • Initial Therapy: Begin with 5/20 mg once daily for 1 to 2 weeks. Your doctor may then titrate the dose as needed, up to a maximum of 10/40 mg once daily.
  • Substituting Components: For patients already on Amlodipine and Olmesartan Medoxomil separately, their individually titrated doses can be substituted with Bizoran. The combination can also be adjusted with increased amounts of Amlodipine, Olmesartan Medoxomil, or both, as necessary.

Special Populations:

  • Elderly Patients (over 75 years): Due to decreased clearance of Amlodipine in this age group, the recommended starting dose of Amlodipine is typically 2.5 mg. Since the lowest dose of Bizoran is 5/20 mg, initial therapy with this combination drug is generally not recommended for patients over 75 years old.
  • Pediatric Use: The safety and effectiveness of Bizoran have not been established in pediatric patients.
  • Renal Impairment: There are no specific studies on Bizoran in patients with renal impairment.
  • Hepatic Impairment: Initial therapy with Bizoran is not recommended in hepatically impaired patients.

Important Considerations & Warnings

It's vital to have a comprehensive discussion with your doctor about your medical history before starting Bizoran, as it can interact with other medications and may not be suitable for certain conditions.

Contraindications:

  • Bizoran cannot be co-administered with Aliskiren in patients with diabetes.

Potential Interactions:

  • The antihypertensive effect of Angiotensin II Receptor Antagonists, including Olmesartan Medoxomil, may be attenuated by NSAIDs, including selective COX-2 inhibitors.
  • Blood pressure, renal function, and electrolytes should be closely monitored in patients on combination therapy with other agents that affect the Renin-Angiotensin System (RAS).

Common Side Effects: The most commonly reported side effects include peripheral edema (swelling, especially in ankles/feet), headache, flushing, and dizziness. It can also cause rare but serious intestinal problems known as sprue-like enteropathy.

Pregnancy & Lactation:

  • Pregnancy Category D: Bizoran should not be used in the 2nd and 3rd trimesters of pregnancy as it can cause fetal death. If pregnancy is detected, this combination should be discontinued as soon as possible.
  • Lactation: It is not known whether Olmesartan and Amlodipine are excreted in human milk. Due to the potential for adverse effects on the nursing infant, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account its importance to the mother.

Precautions & Warnings: Use Bizoran with caution due to the potential risk for:

  • Hypotension in patients with depleted volume or salt.
  • Vasodilation in patients with severe aortic stenosis.
  • Increased frequency, duration, or severity of angina or acute Myocardial Infarction (MI) in patients with severe obstructive coronary artery disease.

Storage Conditions

Do not store Bizoran above 30°C. Keep away from light and out of the reach of children.
