Bygerd 20: Advanced Acid Blockade for Gastric & Duodenal Ulcers and Reflux Esophagitis

Bygerd 20 is a prescription medication indicated for the treatment of various acid-related disorders, including gastric ulcer, duodenal ulcer, and reflux esophagitis. It also plays a crucial role in preventing the recurrence of gastric or duodenal ulcers during low-dose aspirin or NSAID administration. Furthermore, Bygerd is an important adjunct to Helicobacter pylori eradication regimens for a range of conditions, including gastric ulcer, duodenal ulcer, and certain stomach lymphomas.


How Bygerd Works (Pharmacology)

Bygerd contains Vonoprazan, a novel type of acid-suppressing agent known as a potassium-competitive acid blocker (P-CAB). Unlike traditional proton pump inhibitors, Vonoprazan does not require activation by acid. Its mechanism of action involves directly inhibiting the H+, K+-ATPase (proton pump) in gastric parietal cells in a reversible and potassium-competitive manner.

Vonoprazan's strong basicity allows it to accumulate and reside at the acid production site of gastric parietal cells for an extended period, leading to powerful and sustained inhibition of gastric acid production. This strong inhibitory effect significantly reduces mucosal damage in the upper gastrointestinal tract.

Dosage & Administration

Bygerd can be taken without regard to food or timing of food. The dosage varies depending on the condition being treated:

  • Gastric Ulcer: Usual adult dose is 20 mg once daily for 8 weeks.
  • Duodenal Ulcer: Usual adult dose is 20 mg once daily for 6 weeks.
  • Reflux Esophagitis:
    • Initial treatment: 20 mg once daily for 4 weeks. If insufficient, can be extended for up to 8 weeks.
    • Maintenance therapy (for recurrence/recrudescence): 10 mg once daily. If efficacy is inadequate, dosage may be increased up to 20 mg once daily.
  • Prevention of Recurrence of Gastric or Duodenal Ulcer (during low-dose aspirin or NSAID administration): Usual adult dose is 10 mg once daily.
  • Adjunct to Helicobacter pylori Eradication (for adults):
    • First-line regimen (7 days, twice daily): 20 mg Vonoprazan + 750 mg amoxicillin hydrate + 200 mg clarithromycin (clarithromycin dose may be increased to 400 mg twice daily if clinically warranted).
    • Alternative regimen (if clarithromycin-based therapy unsuccessful, 7 days, twice daily): 20 mg Vonoprazan + 750 mg amoxicillin hydrate + 250 mg metronidazole.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking Bygerd, as it has specific contraindications and precautions.

Contraindications:

  • Patients with hypersensitivity to Vonoprazan or any excipient of the product.
  • Patients receiving atazanavir sulphate, nelfinavir, or rilpivirine hydrochloride.

Side Effects: Commonly reported side effects include diarrhea, constipation, nausea, abdominal distension, and edema. Other possible side effects, though less common, can include:

  • Drug hypersensitivity (including anaphylactic shock, drug eruption, urticaria)
  • Hepatotoxicity (liver damage), jaundice, increased liver enzymes (e.g., gamma-glutamyl transferase, AST, ALT, ALP, LDH, y-GPT)
  • Rash
  • Eosinophilia

Precautions & Warnings:

  • Close Observation: The course of the disease should be closely observed, and the minimum therapeutically necessary dose should be used.
  • Long-term Treatment: For long-term use, close observation via endoscopy is recommended.
  • Maintenance Therapy for Reflux Esophagitis: Administer only to patients with recurrent reflux esophagitis who necessitate maintenance of healing; avoid administration to those who do not. Consider dose reduction (to 10mg from 20mg) or suspension of administration when healing is maintained long-term and there is no risk of recurrence.

Use in Special Populations:

  • Elderly: Administer carefully as physiological functions (hepatic/renal) are generally decreased.
  • Children (under 18 years): Safety and efficacy have not been established. Administration is not recommended for this age group.
  • Renal Impairment: No dose adjustment needed for mild to moderate renal impairment (eGFR 30 to 89 mL/min). Avoid use in severe renal impairment (eGFR < 30 mL/min).
  • Hepatic Impairment: No dose adjustment needed for mild hepatic impairment (Child-Pugh A). Avoid use in moderate to severe hepatic impairment (Child-Pugh B or C).

Pregnancy & Lactation:

  • Pregnancy: Use in pregnant women or those with possibility of pregnancy only if the expected therapeutic benefit outweighs any possible risk.
  • Lactation: It is advisable to avoid Bygerd in nursing mothers. If administration is indispensable, nursing should be discontinued.

Overdose Effects: There is no specific information on overdose with Bygerd. It is not removed by hemodialysis. Treatment for overdose should be symptomatic and supportive.


Storage Conditions

Store below 30°C, in a cool and dry place. Keep away from light. Keep all medicine out of the reach of children.
