Cefim-3 (Cefixime) Antibiotic

Indications

Cefim-3 is an orally active third-generation cephalosporin antibiotic used to treat acute infections caused by susceptible microorganisms. It is highly stable in the presence of beta-lactamase enzymes.

Infections treated include:

  • Upper Respiratory Tract Infections (URTI): E.g., otitis media, and other URTI where resistance is suspected.

  • Lower Respiratory Tract Infections: E.g., bronchitis.

  • Urinary Tract Infections: E.g., cystitis, cystourethritis, pyelonephritis.

Pharmacology

Cefixime is a bactericidal antibiotic that kills bacteria by interfering with the synthesis of the bacterial cell wall. Its high stability against Beta-lactamase enzyme means it is effective against many organisms that are resistant to penicillins and some other cephalosporins.

  • Absorption: Approximately 40% to 50% is absorbed, regardless of whether it is taken with or without food.

Dosage and Administration

The usual treatment course is 7 days, which may be extended up to 14 days if needed.

PopulationRecommended Daily DosageNotes
Adults & Children > 10 years200-400 mg dailyGiven as a single dose or in two divided doses.
ElderlySame as adults.Assess renal function and adjust dose for severe impairment.
Children (General)8 mg/kg/dayGiven as a single dose or in two divided doses.
Children (Typhoid)5 mg/kg body weight twice dailyFor 10-14 days.
Severe Renal ImpairmentDose should not exceed 200 mg once daily.(For Creatinine Clearance < 20 ml/min)

The safety and efficacy have not been established in children aged less than 6 months.

Contraindications, Precautions, and Side Effects

  • Contraindications: Patients with known hypersensitivity to cephalosporin antibiotics.

  • Precautions & Warnings:

    • Use with caution in patients sensitive to penicillin, due to potential partial cross-allergenicity.

    • Administer with caution in patients with markedly impaired renal function.

    • Treatment may cause Clostridium difficile overgrowth, leading to antibiotic-associated diarrhea.

    • Care should be taken when given to patients receiving anticoagulant therapy, as increases in prothrombin times have been noted.

  • Side Effects: Generally mild and self-limiting. Most frequent are diarrhea and stool changes. Other effects include nausea, abdominal pain, headache, dizziness, and hypersensitivity reactions (e.g., rash).

  • Pregnancy & Lactation: Should be used only if clearly needed, as adequate human studies are lacking. Use caution in nursing mothers.

Overdose: No specific antidote exists. Treatment should be symptomatic and supportive.

Therapeutic Class: Third generation cephalosporin antibiotic.
