Cefim-3 DS 400mg Capsule is an antibiotic belonging to the cephalosporin group, used to treat a variety of bacterial infections. It is effective against infections of the respiratory tract (e.g., pneumonia), urinary tract, ear, nasal sinus, throat, and some sexually transmitted diseases.

How Cefim-3 DS 400mg Capsule Works (Mode of Action)

Cefim-3 DS 400mg Capsule contains Cefixime, a third-generation semisynthetic cephalosporin antibiotic. It works by killing bacteria by inhibiting the final transpeptidation step of peptidoglycan synthesis in the bacterial cell wall. Peptidoglycan is a crucial component of the bacterial protective covering (cell wall) that is essential for bacterial survival. By binding to one or more of the penicillin-binding proteins (PBPs), Cefixime prevents the bacteria from forming this protective layer, thus inhibiting biosynthesis and arresting cell wall assembly, which ultimately leads to bacterial cell death. Cefixime is highly stable in the presence of Beta-lactamase enzymes, which are produced by some bacteria to resist other antibiotics. This makes it effective against a broad spectrum of Gram-positive and Gram-negative bacteria, including many beta-lactamase producing strains.

Dosage & Administration

Cefim-3 DS 400mg Capsule may be taken with or without food or milk to reduce gastrointestinal discomfort. It should be taken regularly at evenly spaced intervals as prescribed by your doctor. The usual course of treatment is 7 days, which may be extended up to 14 days depending on the severity of the infection.

Adult Dose:

• Acute Bronchitis & Acute Exacerbations of Chronic Bronchitis, Otitis Media, Pharyngitis/Tonsillitis, Uncomplicated Urinary Tract Infections: 200 or 400 mg daily as a single dose or in two divided doses.
• Gonorrhea: 400 mg as a single dose.
• Typhoid fever: 20 mg/kg body weight daily in two divided doses.

Child Dose:

• Children weighing less than 50 kg: 8 mg/kg/day orally, every 12-24 hours. For convalescent oral therapy of serious infections, up to 20 mg/kg/day.
• Children over 12 years: Capsule: 200 or 400 mg daily as a single dose or in two divided doses.
• Powder for Suspension & DS Powder for Suspension & Paediatric Drops (Children above 6 months): 8 mg/kg daily in 1-2 divided doses.
  • 6 months-1 year: 75 mg daily
  • 1-4 years: 100 mg daily
  • 5-10 years: 200 mg daily
• Typhoid Fever (Children): 15-20 mg/kg/day orally divided every 12 hours for 7-14 days; not to exceed 400 mg/day.
• Children less than 6 months: Safety and efficacy have not been established.

Renal Impairment:

• Dose reduction is necessary.
• Creatinine Clearance (CrCl) $<$$20$$ml/min$: Maximum 200 mg daily.

Contraindications:

Side Effects:

• Common: Nausea, stomach pain, indigestion, diarrhea (can occur in 16% of cases). Diarrhea should stop when the course is complete; inform your doctor if it persists or if you find blood in your stools.

• Potentially Fatal: Pseudomembranous colitis (a severe form of diarrhea).

Interactions:

• Probenecid: May increase Cefixime concentrations.
• Anticoagulants (e.g., Warfarin): May increase prothrombin time, potentially leading to increased bleeding risk. This interaction is potentially fatal. Careful monitoring of INR/prothrombin time is recommended if co-administered.
• Alcohol: Avoid consuming alcohol while taking Cefim-3 DS 400mg as it may cause increased side effects.

Pregnancy & Lactation:

• Pregnancy: Available data from published observational studies, case series, and case reports over several decades with cephalosporin use, including Cefixime, in pregnant women have not established drug-associated risks of major birth defects, miscarriage, or adverse maternal or fetal outcomes. Maternal gonorrhea may be associated with adverse outcomes. This medicine is generally regarded as safe to use during pregnancy if prescribed by a doctor.
• Lactation: There are no available data on the presence of Cefixime in human milk. However, it is present in animal milk. The developmental and health benefits of breastfeeding should be considered along with the mother’s clinical need for therapy and any potential adverse effects on the breastfed infant from the drug or from the mother’s underlying condition.
