Indications

Cipcin (Ciprofloxacin) is a broad-spectrum quinolone antibiotic used for treating a wide range of single or mixed infections. It is effective against organisms resistant to other antibiotics, including aminoglycosides, penicillins, and cephalosporins.

• Severe Systemic Infections: Septicemia, bacteremia, and infections in immunosuppressed patients.
• Respiratory Tract Infections: Pneumonia, bronchitis, and cystic fibrosis exacerbations.
• Urinary Tract Infections: Cystitis, pyelonephritis, and prostatitis.
• Skin and Soft Tissue Infections: Infected ulcers, cellulitis, and infected burns.
• Gastrointestinal Infections: Enteric fever and infectious diarrhea.
• Other Infections: Bone and joint infections, pelvic infections, and uncomplicated gonorrhea.

Pharmacology

Ciprofloxacin is a synthetic quinolone that acts by interfering with the bacterial enzyme DNA gyrase, which is essential for DNA synthesis. It is well absorbed orally, widely distributed in the body, and has a half-life of about 3.5 hours. It is primarily excreted unchanged through the kidneys.

Dosage & Administration

The dosage varies based on the infection's severity, type, and patient factors like age, weight, and renal function.

• Oral (Adults):
  • General: 100-750 mg twice daily.
  • Urinary Tract Infections: 250-500 mg twice daily.
  • Gonorrhea: A single dose of 250 or 500 mg.
• Intravenous (IV) Infusion: Dosage ranges from 200 mg to 400 mg, administered every 8 or 12 hours, depending on the infection.
• Renal Impairment: Dosage adjustment may be necessary for severe renal impairment.
• Elderly: No dosage adjustment is typically required.
• Children and Adolescents: Use is generally not recommended due to the risk of joint damage in immature animals, but may be considered if the benefits outweigh the risks (7.5-15 mg/kg/day in two divided doses).
• Duration: Treatment typically lasts 5-10 days and should continue for at least three days after symptoms disappear.

Interaction

• Antacids/Sucralfate: Avoid concurrent use with products containing magnesium, aluminum, calcium, iron, or zinc. Take these products 2 hours after or 6 hours before Cipcin.
• Dairy Products: Do not take with milk or yogurt alone, as it can reduce absorption.

Contraindications

Cipcin is contraindicated in patients with a history of hypersensitivity to Ciprofloxacin or other quinolones.

Side Effects

Common side effects include gastrointestinal issues (nausea, diarrhea, abdominal pain) and CNS disturbances (headache, dizziness, confusion). Other side effects include joint pain and hypersensitivity reactions like skin rashes.

Pregnancy & Lactation

Its use during pregnancy and in nursing mothers is not recommended due to the risk of arthropathy observed in immature animals.

Precautions & Warnings

• Use with caution in patients with CNS disorders like epilepsy.
• Ensure liberal fluid intake to prevent crystalluria.

Overdose Effects

Overdose can lead to seizures, hallucinations, confusion, and renal/hepatic impairment.
