- Urinary tract infections: Cystitis, pyelonephritis, and urethritis.

Dosage & Administration

The dosage varies by the type and severity of the infection.
- Adults (Oral): The dosage range is 100-750 mg twice daily. A single dose of 250-500 mg is used for gonorrhea.
- Adults (IV): Doses vary from 200-400 mg every 8 to 12 hours.
- Renal Impairment: Dosage adjustment is needed for severe renal impairment.
- Children/Adolescents: Use is not recommended due to the risk of arthropathy in immature animals, but a dose of 7.5-15 mg/kg/day in two divided doses may be used if the benefits outweigh the risks.

Contraindications:

- Other: Joint pain, photosensitivity, transient increase in liver enzymes, and a risk of hypoglycemia.
Pregnancy & Lactation:
- Not recommended for use during pregnancy or by nursing mothers due to the potential for arthropathy in the fetus/infant.
Precautions & Warnings:
- Use with caution in patients with CNS disorders.
- Avoid taking with antacids containing magnesium or aluminum, sucralfate, or products containing calcium, iron, or zinc. These should be taken 2 hours after or 6 hours before Civox.
- Do not take concurrently with milk or other dairy products alone.
