Clonium 1 Tablet contains Clonazepam BP 1 mg. It is manufactured by ACI Pharmaceuticals Ltd. in Bangladesh. 

Pharmacology: Clonium (Clonazepam) exerts its antiseizure and antipanic effects by enhancing the activity of gamma-aminobutyric acid (GABA), the major inhibitory neurotransmitter in the central nervous system. 

Indications: Clonium is indicated for the treatment of various conditions, including: 

  • All clinical forms of epileptic disease and seizures in infants, children, and adults, including: 
    • Atypical absence (Petit mal) 
    • Primary or secondary generalized tonic-clonic (grand mal) 
    • Tonic or clonic seizures 
    • Partial (focal) seizures 
    • Myoclonic seizures 
    • Myoclonus 
  • Panic attacks 
  • Bipolar affective disorder 
  • Drug-induced dyskinesia 
  • Choreiform movement 
  • Fulgurant pain 
  • Tourette's syndrome 
  • Resistant depression 
  • Nocturnal myoclonus 
  • Trigeminal neuralgia 

Dosage and Administration: The dosage should be carefully adjusted to individual requirements. 

  • Adults and elderly: Initial dose is 1 mg/day in divided doses (elderly 0.5 mg) at night for 4 nights. The usual maintenance dose is 4-8 mg daily in 3-4 divided doses over 2-4 weeks, according to response. 
  • Infants and children:
    • Up to 1 year:0.25 mg/day in divided doses, increased up to 0.5-1 mg. 
    • Up to 5 years: 0.25 mg/day in divided doses, increased up to 1-3 mg. 
    • 5-12 years: 0.5 mg/day in divided doses, increased up to 3-6 mg. 

Side Effects: The common side effects of Clonazepam are: 

  • Fatigue 
  • Muscle weakness 
  • Dizziness 
  • Lightheadedness 
  • Somnolence (drowsiness) 
  • Hypersalivation in infants 
  • Occasional hypotonia (low muscle tone) 
  • Coordination disturbances 

Precautions:

  • Clonazepam may increase the incidence or precipitate the onset of generalized tonic-clonic seizures when used in patients with multiple types of seizure disorders. 
  • Abrupt withdrawal, especially after long-term, high-dose therapy, may precipitate status epilepticus. 
  • Caution should be exercised in patients with impaired renal function and chronic respiratory diseases. 
  • Clonazepam may produce an increase in salivation, especially in infants. 
  • The addiction/habit-forming potential of Clonium is very high. 
  • It may cause dizziness; avoid driving or operating machinery until you know how it affects you. 
  • Avoid alcohol consumption as it may increase dizziness and drowsiness. 
  • Inform your doctor if you are pregnant, planning to conceive, or breastfeeding. 
  • Inform your doctor if you experience worsening anxiety, depression, angry, or violent behavior and mania while taking this medicine. 

Contraindications: Clonium is contraindicated in patients with: 

  • Known hypersensitivity to benzodiazepines or any other drug excipients. 
  • Acute pulmonary insufficiency. 
  • Severe respiratory insufficiency. 
  • Sleep apnea syndrome. 
  • Myasthenia gravis. 
  • Severe hepatic insufficiency. 
  • Acute narrow-angle glaucoma. 

Drug Interactions: The CNS-depressant action of benzodiazepines may be potentiated by: 

  • Alcohol 
  • Narcotics 
  • Barbiturates 
  • Hypnotics 
  • Antianxiety agents 
  • Phenothiazines, thioxanthenes, and butyrophenone classes of antipsychotic agents 
  • Monoamine oxidase inhibitors (MAOIs) 
  • Tricyclic antidepressants 
  • Other anticonvulsants 

Use in Pregnancy and Lactation: Use of Clonazepam during pregnancy and lactation should be avoided. Clonazepam is excreted into breast milk, and mothers receiving it should not breastfeed. 

Storage: Store in a cool place and protect from light. Keep out of reach of children.
