Comet 500 

Presentation: Comet 500 Tablet, available in Bangladesh, contains Metformin Hydrochloride BP 500 mg.

Indications:Comet is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight patients, when diet and exercise aloneare not sufficient for adequate glycemic control.In adults, it can be used as monotherapy or in combination with other oral antidiabetic agents or with insulin.In children from 10 years of age and adolescents,it can be used as monotherapy or in combination with insulin.Metformin has been shown to reduce diabetic complications in overweight adults with type 2 diabetes when used as first-line therapy after diet failure. It is also used in the treatment of Polycystic Ovary Syndrome (PCOS).  

Dosage and Administration: The usual starting dose for adults is 500 mg two or three times daily or 850 mg once daily, given with meals. Dosage should be increased gradually based on blood glucose response and tolerability, up to a maximum daily dose of 2000-2550 mg, administered in divided doses. For children (10 years and older) and adolescents, the usual starting dose is 500 mg twice daily with meals, with gradual increases up to a maximum of 2000 mg daily in divided doses.  

Side Effects:Common side effects may include gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite.A metallic taste may also occur. These side effects are often transient and may lessen with continued use. Less common but potentially serious side effects include lactic acidosis (more likely in patients with renal impairment) and vitamin B12 deficiency with long-term use.  

Precautions:Comet is contraindicated in patients with known hypersensitivity to metformin or any of its excipients, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute metabolic acidosis, including diabetic ketoacidosis, and conditions that may cause tissue hypoxia (e.g., acute heart failure, severe respiratory disease, recent myocardial infarction).Caution should be exercised in patients with moderate renal impairment, liver disease, excessive alcohol intake, and in elderly patients.Renal function should be assessed before starting treatment and regularly thereafter.
