Comet XR 500 

Presentation:Comet XR 500 Tablet, available in Bangladesh, contains Metformin Hydrochloride BP 500 mgin an extended-release (XR) formulation.It is usually presented as a white to off-white, oval or capsule-shaped tablet for oral administration and is available in blister packs. The "XR" designation indicates a prolonged release of the active ingredient.  

Indications:Comet XR is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight adults, when diet and exercise alone are not sufficient for adequate glycemic control.It can be used as monotherapy or in combination with other oral antidiabetic agents or insulin.Metformin has been shown to reduce the risk of diabetic complications in overweight adults with type 2 diabetes treated as first-line therapy after diet failure.

Dosage and Administration: The usual starting dose is 500 mg once daily with the evening meal. Dosage adjustments should be made in increments of 500 mg weekly based on blood glucose response and tolerability, up to a maximum daily dose of 2000 mg once daily with the evening meal.If glycemic control is not achieved at the maximum once-daily dose, a total daily dose of 2000 mg may be given as 1000 mg twice daily with meals.The tablets should be swallowed whole and not crushed or chewed.  

Side Effects:Common side effects may include gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite, particularly at the beginning of treatment.These symptoms are often transient. Less common but potentially serious side effects include lactic acidosis (more likely in patients with renal impairment) and vitamin B12 deficiency with long-term use.  

Precautions:Comet XR is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), and conditions that may cause tissue hypoxia (e.g., acute heart failure, severe respiratory disease, recent myocardial infarction).Caution should be exercised in patients with moderate renal impairment, liver disease, excessive alcohol intake, and in elderly patients. Renal function should be assessed before starting treatment and regularly thereafter.
