Indications:
Comfy is indicated for the treatment of:
- Panic disorder, with or without agoraphobia.
- Lennox-Gastaut Syndrome (petit mal variant), akinetic, and myoclonic seizures, alone or as an adjunct.
- Absence seizures (petit mal)in patients who have failed to respond to succinimides.
Pharmacology:
Clonazepam, the active ingredient in Comfy, belongs to the benzodiazepine class of drugs.Benzodiazepines are believed to work by enhancing the effects of gamma-aminobutyric acid (GABA), a neurotransmitter that inhibits activity in the brain.This action results inanticonvulsant, sedative, anxiolytic, and muscle relaxant effects.
Dosage and Administration:
Dosage should be individualized by the physician.
- Adults:
- Seizure disorders: The initial dose should not exceed 1.5 mg/day, administered orally in three divided doses. The dosage may be increased in increments of 0.5 to 1 mg every 3 days until seizures are adequately controlled or until side effects precludefurther increase.The maximum recommended daily dose is 20 mg.
- Panic disorder:The initial dose is 0.25 mg, administered orally twice daily. After 3 days, the dose may be increased to 1 mg/day (0.5 mg twice daily). Subsequent dose increases may be made in increments of 0.125 to 0.25 mg twice daily every 3 days until panic disorder is controlled or until side effects make further increasesunwarranted. The maximum recommended daily dose is 4 mg.
- Pediatric Patients: The initial daily dose for infants and children (up to 10 years or 30 kg body weight) should be between 0.01 and 0.03 mg/kg/day, administered in two or three divided doses. The dosage should not exceed 0.05 mg/kg/day. The dosage may be increased by 0.25 to 0.5 mg every 3 days until a daily maintenance dose of 0.1 to 0.2 mg/kg of body weight has been reached, unless seizures are controlled or side effects preclude further increase.
Side Effects:
The most frequently occurring side effects of Comfy are related to central nervous system (CNS) depression. These include:
- Drowsiness
- Ataxia (impaired coordination)
Other possible side effects, with varying frequency, include:
- Behavioral problems
- Abnormal eye movements
- Aphonia (loss of voice)
- Coma
- Tremor
- Vertigo (dizziness)
- Confusion
- Depression
- Amnesia (memory loss)
- Hallucinations
- Hysteria
- Increased libido (sexual desire)
- Insomnia (difficulty sleeping)
- Psychosis
- Palpitations
Precautions and Warnings:
- Comfy may increase the incidence or precipitate the onset of generalized tonic-clonic seizures in patients with mixed seizure disorders.
- Special attention to maintaining open airways is important in infants and small children, as Comfy can increase the production of saliva and bronchial secretions.
- Benzodiazepine pharmacologic effects appear to be greater in elderly patients compared to younger patients. Use lower doses in the elderly.
- As with other benzodiazepines, Comfy has the potential to cause dependence and withdrawal symptoms upon discontinuation. Gradual withdrawal is recommended.
- May cause drowsiness and impair the ability to drive or operate machinery. Avoid alcohol consumption while taking Comfy.
- Monitor patients for the emergence or worsening of depression, suicidal thoughts or behavior, and/or any unusual changes in mood or behavior.
Contraindications:
Comfy should not be used in patients with:
- A history of hypersensitivity to benzodiazepines.
- Clinical or biochemical evidence of significant liver disease.
- Acute narrow-angle glaucoma.
Drug Interactions:
- Comfy does not appear to alter the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital. However, caution is advised when these drugs are used concomitantly.
- The combination of Comfy and zolpidem or quetiapine should be avoided as it may increase the chances of side effects. The CNS-depressant effects of benzodiazepines may be additive with other CNS depressants, including alcohol, barbiturates, narcotics, and other sedatives or tranquilizers.
Use in Pregnancy and Lactation:
The use of Comfy during pregnancy should generally be avoided, especially during the first and third trimesters, due to potential risks to the fetus. Clonazepam is excreted in breast milk, and breastfeeding is not recommended while taking this medication.
Storage:
Store Comfy tablets in a cool, dry place, protected from light. Keep out of reach of children.
