Convules CR 300 Tablet is a controlled-release formulation containing Sodium Valproate 300 mg.It is manufactured by Opsonin Pharma Limited in Bangladesh.  

Indications:

Convules CR is indicated for the treatment of various types of epilepsy, including:

  • Partial seizures  
  • Absence seizures (petit mal)  
  • Generalized tonic-clonic seizures (grand mal)  
  • Myoclonic seizures  
  • Atonic seizures  
  • Mixed seizures that include absence attacks  
  • Prophylaxis of febrile convulsions
  • Prophylaxis of post-traumatic epilepsy  

It is also indicated in the treatment of bipolar disorder and prophylaxis of migraine.  

Pharmacology:

Sodium Valproate is an antiepileptic drug with a broad spectrum of activity against various types of seizures. Its exact mechanism of action is not fully established, but it is thought to increase brain levels of gamma-aminobutyric acid (GABA), an inhibitory neurotransmitter in the brain.

Dosage and Administration:

The dosage should be individualized and determined by the physician. The controlled-release formulation allows for once or twice-daily administration. It is usually taken after food.

  • Adults:The usual starting dose is 600 mg daily in 2 divided doses, preferably after food. The dose may be increased by 200 mg/day at 3-day intervals until seizure control is achieved, up to a maximum of 2.5 g daily in divided doses. The usual maintenance dose is 1-2 g daily (20-30 mg/kg daily).  
  • Children (over 20 kg):Initially 400 mg daily in divided doses, increased until controlis achieved (usually in the range of 20-30 mg/kg daily), with a maximum of 35 mg/kg daily.  
  • Children (up to 20 kg): Initially 20 mg/kg daily in divided doses, may be increased provided plasma concentrationsare monitored (above 40 mg/kg daily also monitor clinical chemistry and hematological parameters).  
  • Febrile convulsion: 20-30 mg/kg/day in 3 divided doses.  
  • Bipolar disorder:Initially 20-30 mg/kg/day in 2-3 divided doses; adjust dosage in 3-5 days.The maintenance dosage is 1000-2000 mg daily.  
  • Prophylaxis of migraine:300 mg twice daily, although some may require 1000 mg daily.  

Side Effects:

The most common side effects are:

  • Anorexia
  • Nausea  
  • Vomiting  

These side effects are often minimized with the use of enteric-coated tablets. Other side effects may include:

  • Sedation  
  • Ataxia (impaired coordination)  
  • Tremor  
  • Rash  
  • Alopecia (hair loss)  
  • Stimulation of appetite  
  • Weight gain
  • Increased bleeding time
  • Thrombocytopenia (low platelet count)  
  • Headache  
  • Sleepiness  
  • Gum swelling  
  • Liver injury (rare but serious)

Precautions:

  • Liver function should be monitored before therapy and during the first 6 months, especially in high-risk patients.  
  • Care should be taken in patients with renal impairment and systemic lupus erythematosus.  
  • Avoid sudden withdrawal of therapy.
  • Use with caution during pregnancy and breastfeeding.
  • May cause drowsiness and affect the ability to drive or operate machinery.
  • Alcohol consumption should be avoided while taking this medication.

Contraindications:

Convules CR is contraindicated in patients with:

  • Known hypersensitivity to sodium valproate.
  • Liver dysfunction or a family history of liver problems.
  • Urea cycle disorders.
  • Mitochondrial disorders (e.g., Alpers-Huttenlocher syndrome) in children under 2 years.

Drug Interactions:

Sodium Valproate can interact with several drugs, including:

  • Phenobarbital
  • Phenytoin
  • Warfarin
  • Aspirin
  • Antidepressants (SSRIs and related antidepressants)
  • Carbamazepine
  • Lamotrigine
  • Zidovudine
  • Mefloquine
  • Bupropion

Use in Pregnancy and Lactation:

Sodium Valproate is contraindicated during pregnancy due to the risk of neural tube defects and other congenital malformations.It is excreted in breast milk, but breastfeeding is generally considered to pose low risk to the infant. However, consult your doctor for specific advice.

Storage:

Store in a cool and dry place, protected from light. Keep out of reach of children.
