Cytomis 200: Uses, Mechanism, and Important Considerations

Cytomis 200 is a medication primarily used for medical abortion and for preventing excessive bleeding after delivery (postpartum hemorrhage). It contains Misoprostol, a synthetic prostaglandin E1 analogue. 

How Cytomis 200 Works (Mode of Action & Pharmacology)

Cytomis 200 (Misoprostol) works by: 

• Increasing uterine contractions: This action is central to both its uses. By stimulating the uterus to contract, it helps to expel the contents in a medical abortion and to prevent excessive bleeding after childbirth by ensuring the uterus contracts properly. 
• Antisecretory and Cytoprotective Effects (for ulcers): Misoprostol directly acts on specific prostaglandin receptors on gastric parietal cells to exert its antisecretory activity, reducing stomach acid production. It also has protective effects on the stomach lining by replacing prostaglandins consumed during therapies like NSAID use, thereby preventing and treating NSAID-induced ulcers. 

Key Indications & Benefits

Cytomis 200 is indicated for: 

• Medical Abortion (Terminating a pregnancy): Often used in combination with another medicine (mifepristone) to achieve maximum benefit. 
• Prevention of Post-Delivery Bleeding (Postpartum Hemorrhage): Due to poor contraction of the uterus. 
• NSAID-Induced Ulcers: Prevention and treatment of gastric and duodenal ulcers caused by non-steroidal anti-inflammatory drugs (NSAIDs). 
• Benign Gastric and Duodenal Ulcers: Treatment of existing ulcers. 
• Induction of Labor: Used to ripen the cervix and induce labor. 

Dosage & Administration

Cytomis 200 should be taken with food or as advised by your doctor. Swallow tablets whole with water. If vomiting occurs within 30 minutes, inform your doctor or take another tablet. 

Specific Adult Doses:

• NSAID-Induced Ulcer (Treatment): 800 mcg daily (in 2-4 divided doses) with meals and at bedtime for at least 4 weeks, up to 8 weeks. 
• NSAID-Induced Ulcer (Prophylaxis): 200 mcg 2-4 times daily with NSAID. If not tolerated, 100 mcg can be used for the duration of NSAID therapy. 
• Induction of Labor: 100 mcg orally. If labor does not occur, repeated doses of 100-200 mcg orally every 4 hours until labor is established (max 6 doses). 
• Termination of Pregnancy:
  • Day 1: 200 mg mifepristone orally (single dose under physician supervision). 
  • Days 2-3: 800 mcg misoprostol buccally once (single dose), 24-48 hours after mifepristone. 
  • Days 7-14: Follow-up visit to confirm complete termination. If incomplete but not ongoing pregnancy, another 800 mcg buccally with follow-up in ~7 days. Surgical evacuation is recommended for ongoing pregnancies after medical abortion. 
• Prevention of Postpartum Hemorrhage: 600 mcg orally immediately following delivery. 

Renal Impairment: Use with caution; peak plasma concentration, half-life, and bioavailability may be increased. 

Important Considerations & Warnings

Contraindications:

• History of allergy to prostaglandins. 
• Pregnancy and lactation (for indications other than medical abortion). 
• Women of childbearing potential (unless requires NSAID therapy and effective contraception is used, or for medical abortion under strict medical supervision). 

Side Effects:

• Common (>10%): Diarrhea (14-40%), Abdominal pain (13-20%). 
• 1-10%: Headache (2%). 
• Frequency Not Defined: Nausea, vomiting, uterine cramps, menorrhagia (heavy menstrual bleeding), flatulence, gastrointestinal hemorrhage, chest pain, cardiac dysrhythmia, myocardial infarction, anaphylaxis, anemia, hearing loss, rupture of uterus (rare but serious, especially during labor induction or in women with previous uterine surgery), thromboembolic disorder. 

Interactions:

• May increase effects of oxytocin. 
• Increased risk of misoprostol-induced diarrhea with magnesium-containing antacids. 

Pregnancy & Lactation:

• Pregnancy: Cytomis 200 is used for medical abortion. For other indications (like ulcer prevention), it is absolutely contraindicated in pregnant women as it can cause miscarriage, premature birth, or birth defects. Women of childbearing potential must use effective contraception while taking misoprostol for ulcer prevention. 
• Lactation: Misoprostol acid is excreted into breast milk. Although no published reports of adverse effects in breast-feeding infants exist, caution should be exercised when administered to breastfeeding women. 

Precautions & Warnings:

• Pregnancy status: For NSAID-induced ulcer prevention, confirm the patient is not pregnant and advise effective contraception. 
• Hypotension: Use with caution in conditions where low blood pressure might cause severe complications (e.g., cerebrovascular or cardiovascular disease). 
• Other conditions: Inflammatory bowel disease, patients prone to dehydration, elderly patients, renal impairment. 
• Vaginal Bleeding: Bleeding may last for up to 12 days after medical abortion. Inform your doctor if it lasts longer or if you experience severe abdominal pain. 
• Activity Restrictions (during abortion): Avoid strenuous activity, heavy exercise, running, and driving. 
• Follow-up (after abortion): Your doctor may perform an ultrasound or pelvic examination to confirm abortion completion. Lack of bleeding usually indicates failure.
