D-Rise 20000 is primarily used for the treatment and prevention of Vitamin D3 deficiency. It's crucial for:  

  • Bone Health: Essential for the absorption of calcium and phosphate, which are vital for strong bones and teeth. It helps prevent conditions like rickets (in children), osteomalacia (softening of bones in adults), and osteoporosis (weak and brittle bones).  
  • Immunity: Plays a vital role in boosting the immune system.  
  • Pregnancy & Lactation: Important for preventing pre-eclampsia during pregnancy and for the healthy development of a growing infant.  
  • Adjunct Therapy: Can be used as an addition to specific treatments for osteoporosis, hypocalcaemia (low calcium), and tetany (muscle spasms).  

 

How D-Rise 20000 Works (Pharmacology)

Doxycycline Hydrochloride is a semisynthetic tetracycline antibiotic. Its primary mechanism of action is bacteriostatic, meaning it inhibits the growth and reproduction of bacteria rather than directly killing them. Inside the bacterial cell, Doxycycline interferes with protein synthesis, which is essential for bacterial growth and replication.  

 

Doxycycline has a broad spectrum of activity against:  

  • Gram-positive and Gram-negative bacteria, including strains resistant to other tetracyclines (e.g., certain Staphylococcus aureus, E. coli, Proteus mirabilis, and Klebsiella).  
  • Anaerobes, with good activity against Bacteroides fragilis.  
  • Spirochetes (e.g., Borellia recurrentis, Treponema pallidum, Treponema pertenue).  
  • Mycoplasma, Rickettsia, Chlamydia, Actinomyces, Brucella, and Vibrio cholerae.  
  • It is also active against Plasmodium falciparum (a malaria parasite) and Nocardia.  
  • It is particularly useful for infections with H. ducreyi (causative agent of chancroid) and Haemophilus influenzae.  

 

Dosage & Administration

 

The specific dosage and administration of D-Rise 20000 will depend on your age, the severity of your Vitamin D deficiency, and your doctor's recommendation. D-Rise 20000 IU is typically available in capsule form.  

Common Oral Dosages (Capsules):

  • For prevention of Vitamin D3 deficiency (Adults): 20,000 IU every 4 weeks. Higher doses may be required in certain situations.  
  • For treatment of Vitamin D3 deficiency (Children 12-18 years): 20,000 IU once every 2 weeks for 6 weeks.  
  • For prevention of Vitamin D3 deficiency (Children 12-18 years): 20,000 IU every 6 weeks.  
  • As an adjunct to specific therapy for osteoporosis (Adults): 20,000 IU once a month.  

It is generally recommended to take Vitamin D3 supplements with food or within 1 hour after a meal to enhance absorption, as Vitamin D is fat-soluble. Capsules should be swallowed whole with plenty of fluid.

Important Considerations & Warnings

 

It is crucial to discuss your full medical history with your doctor before taking D-Rise 20000.  

Contraindications:

  • All diseases associated with hypercalcaemia (high calcium levels in the blood).  
  • Known hypersensitivity to Vitamin D3 or any of its components.  
  • Confirmed Vitamin D3 toxicity (hypervitaminosis D).  
  • Nephrolithiasis (kidney stones).  
  • Severe renal impairment.  

Side Effects (Rarely occur, usually with excessive intake):

  • Related to high calcium levels (hypercalcaemia): Anorexia, lassitude (lack of energy), nausea, vomiting, diarrhea, constipation, weight loss, polyuria (frequent urination), sweating, headache, thirst, vertigo.  
  • Elevated concentrations: Increased calcium and phosphate levels in plasma and urine.  
  • Other rare effects: Skin rash, pruritus (itching), urticaria (hives).  

Pregnancy & Lactation:

  • Pregnancy: Studies suggest safe use of doses up to 4,000 IU during pregnancy. However, higher doses for deficient women should be under medical guidance. Always follow your doctor's advice.  
  • Lactation: Vitamin D3 and its metabolites are excreted in breast milk. While infant overdose via breast milk is not commonly observed, your doctor should consider the mother's dose if prescribing additional Vitamin D3 to a breastfed child.  

Precautions & Warnings:

  • Monitor calcium levels: Plasma-calcium concentration should be monitored at intervals, especially in patients receiving high doses of Vitamin D3, those with renal impairment, and during pregnancy and lactation.  
  • Drug interactions:
    • Magnesium-containing antacids, Digoxin, Thiazide Diuretics, Cholestyramine, Cholistipol, Phenytoin, Phenobarbital, Orlistat, Mineral Oil: These drugs can interfere with Vitamin D3 absorption and metabolism, potentially leading to deficiency.  
    • Corticosteroids (e.g., Prednisolone): Can increase the body's need for Vitamin D3.  
    • Digoxin and Thiazide Diuretics: Use with caution and consult a healthcare professional due to potential risks (e.g., increased risk of digitalis toxicity with Digoxin in presence of hypercalcaemia, or increased risk of hypercalcaemia with Thiazide Diuretics).  
  • Underlying conditions: Patients with liver or kidney disease, primary hyperthyroidism, lymphoma, tuberculosis, and granulomatous disease should consult a healthcare practitioner before supplementing with Vitamin D3.  
  • Children under 12 years: The safety and efficacy of high-dose Vitamin D3 (like 20000 IU) in children under 12 years have not been fully established for all formulations, though specific syrup dosages exist for younger children.  

Overdose Effects:

  • Overdose can lead to hypervitaminosis D, characterized by symptoms related to high calcium levels in the blood, such as severe cardiac arrhythmias and kidney damage in severe cases.
