DDR 30mg Capsule: An Acid Reducer for Stomach and Esophageal Conditions

DDR 30mg Capsule is a medicine that reduces the amount of acid produced in your stomach. It belongs to a class of medicines known as proton pump inhibitors (PPIs). It is primarily used to treat acid-related diseases of the stomach and intestine and to prevent stomach ulcers.


How DDR 30 Works (Mode of Action & Pharmacology)

DDR 30 contains Dexlansoprazole, which is the R-enantiomer of lansoprazole. As a proton pump inhibitor (PPI), it works by irreversibly binding to the H+/K+-exchanging ATPase (proton pump) in the gastric parietal cells of the stomach. This action effectively blocks the final step of acid secretion, resulting in a significant reduction in stomach acid production.

DDR 30 uses a dual-release formulation, which allows for extended and sustained acid suppression. This helps in the relief of acid-related indigestion and heartburn.


Key Indications & Benefits

DDR 30 is indicated for:

  • Gastroesophageal Reflux Disease (GERD) / Acid Reflux:
    • Healing of all grades of erosive esophagitis (EE).
    • Maintaining healing of EE.
    • Treating heartburn associated with symptomatic non-erosive GERD.
  • Peptic Ulcer Disease: Used for treating benign gastric and duodenal ulcers.
  • Prevention of NSAID-induced Ulcers: To prevent stomach ulcers and acidity that may occur with prolonged use of pain-killers (Non-Steroidal Anti-Inflammatory Drugs).
  • Oesophagitis: Inflammation of the esophagus.
  • Other stomach conditions associated with excessive acid production.

Dosage & Administration

DDR 30mg Capsule should be taken empty stomach, preferably one hour before a meal, in the morning. Do not chew, crush, or break the capsule. The dose and duration will depend on your condition and response.

Adult Dose:

  • Erosive Esophagitis (Healing): 60 mg orally once daily for up to 8 weeks.
  • Erosive Esophagitis (Maintenance): 30 mg orally once daily for up to 6 months.
  • Gastroesophageal Reflux Disease (Symptomatic non-erosive GERD): 30 mg orally once daily for 4 weeks.

Child Dose ( > 12 years):

  • Erosive Esophagitis (Healing): 60 mg orally once daily for up to 8 weeks.
  • Erosive Esophagitis (Maintenance): 30 mg orally once daily for up to 6 months.
  • Gastroesophageal Reflux Disease (Symptomatic non-erosive GERD): 30 mg orally once daily for 4 weeks.
  • <12 years: Safety and efficacy have not been established.

Hepatic Impairment:

  • Mild (Child-Pugh A): No dose adjustment necessary.
  • Moderate (Child-Pugh B): Not to exceed 30 mg/day.
  • Severe (Child-Pugh C): Not recommended.

Missed Dose: If you miss a dose, take it as soon as possible. If it's almost time for your next dose, skip the missed one and continue your regular schedule. Do not double the dose.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking DDR 30.

Contraindications:

  • Hypersensitivity to dexlansoprazole, lansoprazole, or any components of the product.

Side Effects:

  • Common (1-10%): Diarrhea (5%), abdominal pain (4%), nausea (3%), upper respiratory infection (2-3%), vomiting (1-2%), flatulence (1%).
  • Less Common (<1% selected): Arrhythmia, bradycardia, Barrett's esophagus, DVT, dyspnea, hepatomegaly, hypertension, paresthesia, rectal hemorrhage, vulvovaginal infection.
  • Long-term use risks: May increase the risk of bone fractures (especially with higher doses for >1 year) and deficiency of minerals like magnesium (hypomagnesemia).

Interactions:

  • Increased risk of hypomagnesemia: With diuretics and digoxin.
  • Decreased plasma concentration/bioavailability of: Erlotinib, dasatinib, lapatinib, itraconazole, ketoconazole.
  • Increased plasma concentration of: Cilostazol, methotrexate.
  • Reduced bioavailability with: Antacids and sucralfate (take at least 30 minutes before meal or adjust timing).
  • Potentially Fatal: May decrease serum levels and pharmacological effects of rilpivirine and atazanavir (HIV medications). Avoid concomitant use.

Pregnancy & Lactation:

  • Pregnancy Category Note: There are no adequate studies on dexlansoprazole in pregnant women. However, observational studies on lansoprazole (the parent compound) did not show an association with adverse pregnancy outcomes. Animal studies showed reductions in offspring bone development at high doses. Advise pregnant women of potential risks.
  • Lactation: No information on dexlansoprazole in human milk. Lansoprazole and its metabolites are present in rat milk. Consider the benefits of breastfeeding against potential adverse effects on the infant or the mother's underlying condition. Consult your doctor.

Precautions & Warnings:

  • Gastric Malignancy: Gastric malignancy should be ruled out before starting treatment, as PPIs can mask symptoms of stomach cancer.
  • Hepatic Impairment: Use with caution, especially in moderate impairment.
  • Bone Loss (Osteoporosis): Discuss ways to prevent bone loss (e.g., calcium and vitamin D supplements) with your doctor, especially with long-term use.
  • Kidney Problems: Consult your doctor immediately if you develop decreased urination, edema, lower back pain, nausea, fatigue, rash, or fever, as these could be signs of a kidney problem.
  • Driving/Concentration: Generally well-tolerated, but some side effects like dizziness could affect these activities.

Keep below 30°C temperature, away from light & moisture. Keep out of the reach of children.
