Dosage and Administration:
Pharmacology: Domperidone is a peripheral dopamine-receptor blocker. It increases oesophageal peristalsis, enhances gastric emptying and duodenal peristalsis and regulates contraction of the pylorus. It has little effect on dopaminergic receptors in the brain, thus lacking psychotropic and neurologic side effects.
Side Effects: Side effects are uncommon. Occasionally, transient intestinal cramps may occur. Extrapyramidal reactions are rare in young children and exceptionally rare in adults. These symptoms resolve spontaneously and completely upon discontinuation of treatment. Domperidone may increase prolactin levels, potentially leading to galactorrhoea and gynaecomastia in rare cases. Dry mouth, thirst, headache, nervousness, drowsiness, diarrhoea, skin rash and itching have been reported rarely.
Precautions: Should be used with caution in young children due to a potentially increased risk of extrapyramidal reactions because of an incompletely developed blood-brain barrier. Use with caution in patients with hepatic impairment. In patients with severe renal insufficiency, the dosing interval should be increased to once or twice daily depending on the severity of the impairment.
Contraindications: Hypersensitivity to domperidone. Should not be used whenever gastrointestinal stimulation might be dangerous (e.g., gastrointestinal haemorrhage, mechanical obstruction or perforation). Contraindicated in patients with prolactin-releasing pituitary tumour (prolactinoma).
Pregnancy & Lactation: The safety of domperidone has not been proven, and it is therefore not recommended during pregnancy unless the anticipated therapeutic benefit justifies the potential risk. Domperidone may precipitate galactorrhoea and improve post-natal lactation. It is secreted in breast milk in very small quantities insufficient to be considered harmful. Â
