Ecosprin 75 is a tablet containing Aspirin BP 75 mg. It is manufactured by The ACME Laboratories Ltd. in Bangladesh.  

Indications:

Ecosprin 75 is primarily an antiplatelet medication used to prevent blood clots. Its main indications include:  

  • Prevention of heart attacks (myocardial infarction)
  • Prevention of strokes (ischemic stroke/transient ischemic attack - TIA)  
  • Prevention of re-infarction after a previous heart attack  
  • Prevention of arterial occlusive events after bypass surgery or in patients with established peripheral arterial disease.  
  • It may also be used for mild to moderate pain (e.g., headache, muscle pain, dysmenorrhea, toothache) and as an antipyretic (fever reducer), though its primary use at this dose is antiplatelet.  

Pharmacology:

Aspirin (Acetylsalicylic Acid) works by irreversibly inhibiting cyclooxygenase (COX) enzymes, particularly COX-1, in platelets. This inhibition prevents the formation of thromboxane A2, a potent inducer of platelet aggregation and vasoconstriction. By inhibiting platelet aggregation, Aspirin helps prevent the formation of blood clots in arteries, which can lead to heart attacks and strokes. At lower doses like 75 mg, its antiplatelet effect is dominant.  

Dosage and Administration:

Ecosprin 75 is typically taken once daily. It should generally be taken with food to minimize stomach upset.  

  • For prophylaxis of myocardial infarction/stroke/arterial occlusive events: 75 mg once daily.  
  • For acute coronary syndrome (e.g., suspected heart attack): A loading dose of 150-300 mg may be taken immediately, followed by daily maintenance.  
  • After myocardial infarction: 150 mg daily for one month, then 75 mg daily long-term.  
  • After acute ischemic stroke/TIA: 150-300 mg daily initially, then 75 mg daily.  

Contraindications:

Ecosprin 75 is contraindicated in:  

  • Children under 12 years (due to the risk of Reye's syndrome).  
  • Patients with known hypersensitivity to Aspirin, other salicylates, or NSAIDs.  
  • Active peptic ulceration or a history of stomach/intestinal bleeding.  
  • Bleeding disorders (e.g., hemophilia, hemorrhagic disorders, low platelet count).  
  • Gout (may precipitate attacks).  
  • Severe renal or hepatic impairment.  
  • Third trimester of pregnancy.  
  • Breastfeeding mothers.  

Side Effects:

Common side effects include:  

  • Gastrointestinal disturbances: Indigestion, nausea, vomiting, heartburn, epigastric discomfort, stomach upset.  
  • Increased bleeding risk: Easy bruising, prolonged bleeding time.  
  • Hypersensitivity reactions: Hives (urticaria), rash, runny nose, bronchospasm (difficulty breathing), angioedema.  

More serious, but rare, side effects can include:  

  • Gastrointestinal bleeding (bloody or tarry stools, coughing up blood).  
  • Ringing in the ears (tinnitus) or hearing difficulty (especially at higher doses).  
  • Liver or kidney problems.  
  • Severe allergic reactions.  

Precautions and Warnings:

  • Use with caution in patients with a history of asthma, nasal polyps, uncontrolled hypertension, or a history of peptic ulcers (even if not active).  
  • Inform your doctor if you have liver or kidney disease.  
  • Discontinue use and seek medical attention if you experience unusual bleeding, black/tarry stools, or ringing in your ears.  
  • Do not stop taking Ecosprin 75 suddenly, especially if prescribed for heart conditions, without consulting your doctor.  

Pregnancy and Lactation:

  • Pregnancy: Generally classified as Pregnancy Category C, and Category D in the third trimester. It is especially crucial not to take Aspirin during the last three months of pregnancy unless specifically directed by a doctor, as it can harm the unborn child or cause complications during delivery.  
  • Lactation: Aspirin and its metabolites pass into breast milk in small quantities. Its use during breastfeeding is generally not recommended, and a decision should be made in consultation with a doctor.  

Drug Interactions:

  • Anticoagulants (e.g., Warfarin, Heparin) and other antiplatelet drugs (e.g., Clopidogrel): Increased risk of bleeding.  
  • NSAIDs (e.g., Ibuprofen, Naproxen): Increased risk of gastrointestinal ulcers and bleeding. Ibuprofen may also reduce the antiplatelet effect of Aspirin.  
  • Alcohol: Increased risk of gastrointestinal bleeding.  
  • Corticosteroids: Increased risk of GI ulceration.  
  • Methotrexate: Aspirin may increase methotrexate levels and toxicity.  
  • Oral hypoglycemics (e.g., Sulfonylureas, Insulin): Large doses of Aspirin may enhance their blood sugar-lowering effect.  
  • Certain antidepressants (SSRIs): Increased risk of upper gastrointestinal bleeding.  

Storage Conditions:

Store in a cool and dry place, protected from light. Keep out of reach of children.
