Presentation:Each tablet of Empamet 500 contains 5 mg of empagliflozin and 500 mg of metformin hydrochloride.
Indications:Empamet is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhen treatment with both empagliflozin and metforminis appropriate.It is used in patients whose blood sugar is not controlled well enough with metformin alone or who are already taking empagliflozin and metformin as separate tablets.Empagliflozin, one of the components of Empamet, is also indicated to reduce the risk of cardiovascular death in adults with type 2 diabetes mellitus and established cardiovascular6disease.
Dosage and Administration: The dosage of Empamet should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommendeddaily dose of 25 mg of empagliflozin and 2000 mg of metformin hydrochloride. It is usually taken twice daily with meals to reduce the gastrointestinal side effects associated with metformin.
Side Effects:Common side effects may include urinary tract infections, female genital mycotic infections, nausea, vomiting, diarrhea, abdominal discomfort, and headache.Hypoglycemia may occur when Empamet is used in combination with insulin or sulfonylureas. Increased urination and thirst may also occur due to the empagliflozin component.
Precautions:Empamet is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), end-stage renal disease, dialysis, acute metabolic acidosis, including diabetic ketoacidosis, and known hypersensitivity to empagliflozin, metformin, or any of the excipients.It is not recommended for use in patients with type 1 diabetes mellitus. Assess renal function before initiation and monitor periodically. Avoid excessive alcohol intake. Metformin should be temporarily discontinued in patients undergoing radiologic studies with intravascular administration of iodinated contrast materials or any surgical procedures necessitating restricted intakeof food and fluids. An increased risk of diabetic ketoacidosis has been reported with SGLT2 inhibitors, including empagliflozin.Rare cases of Fournier's gangrene (necrotizing fasciitis of the perineum) have been reported with SGLT2 inhibitors.
