Empatab M

Presentation: Empatab M tablets are film-coated and contain a combination of empagliflozin and metformin hydrochloride.

Indications: Empatab M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both empagliflozin and metformin is appropriate. Empagliflozin, one of its components, is also indicated to reduce the risk of cardiovascular death in adults with type 2 diabetes mellitus and established cardiovasculardisease, and to reduce the risk of cardiovascular death and hospitalization for heart failure in adults with heart failure.

Dosage and Administration: The dosage of Empatab M is individualized based on the patient's current regimen. It is usually taken once or twice daily with meals, depending on the formulation (immediate-release or extended-release). The maximum recommended daily dose is 25 mg of empagliflozin and 2000 mg of metformin hydrochloride. Dose escalation should be gradual to reduce gastrointestinal side effects associated with metformin. Renal function should be assessed before initiating Empatab M and periodically thereafter. It is contraindicated in patients with an eGFR below 45 mL/min/1.73 m².

Side Effects:

  • Very Common: Hypoglycemia (when used with insulin or sulfonylureas), gastrointestinal disturbances (diarrhea, nausea, vomiting, flatulence, abdominal discomfort, indigestion).
  • Common: Urinary tract infections, female genital mycotic infections, increased urination, increased thirst, altered taste, headache, asthenia.
  • Uncommon: Dehydration, hypotension, dizziness, increased serum creatinine, reduced glomerular filtration rate, increased hematocrit.
  • Rare: Diabetic ketoacidosis, Vitamin B12 deficiency.

Precautions: Empatab M is contraindicated in patients with severe renal impairment, end-stage renal disease, or dialysis, acute or chronic metabolic acidosis (including diabetic ketoacidosis),and hypersensitivity to empagliflozin or metformin. It should be used with caution in patients with moderate renal impairment, heart failure, and hepatic impairment. Alcohol consumption should be limited due to the increased risk of lactic acidosis associated with metformin. Volume status should be assessed and corrected in patients at risk of volume depletion.

Storage: Store below 30°C, protected from light and moisture. Keep out of reach of children.
