Linagliptin (DPP-4 Inhibitor): Increases active incretin levels (GLP-1 and GIP) to boost glucose-dependent insulin release and reduce glucagon production.
Empagliflozin (SGLT2 Inhibitor): Blocks kidney glucose reabsorption, prompting excess glucose to be excreted in urine.
Starting Dose: Empagliflozin 10 mg / Linagliptin 5 mg once daily in the morning (with or without food).
Maximum Dose: Empagliflozin 25 mg / Linagliptin 5 mg once daily.
Kidney Function: Assess renal function before starting. Do not use if eGFR falls below 45 mL/min/1.73 m².
Contraindications: Severe renal impairment, ESRD, dialysis, or known hypersensitivity to either ingredient.
Pregnancy: Not recommended, particularly during the 2nd and 3rd trimesters.
Serious Adverse Risks: Pancreatitis, diabetic ketoacidosis (DKA), Fournier’s gangrene, severe UTIs (urosepsis), volume depletion/hypotension, and heart failure.
Other Side Effects: Genital mycotic infections, hypoglycemia (if combined with insulin), joint pain, and bullous pemphigoid.
Diuretics: Increases dehydration and low blood pressure risk.
Insulin/Secretagogues: Heightens hypoglycemia risk.
Rifampin (P-gp/CYP3A4 Inducers): Can reduce Linagliptin effectiveness.
Lab Interference: Results in positive urine glucose tests and unreliable 1,5-AG assay measurements.
