Epicet XR 750

Presentation: Each extended-release tablet contains Levetiracetam INN 750 mg. Available in strips.

Indications: Epicet XR is indicated as:

• Monotherapy in the treatment of partial-onset seizures with or without secondary generalization in adults and adolescents from 16 years of age with newly diagnosed epilepsy.
• Adjunctive therapy in the treatment of:
  • Partial-onset seizures with or without secondary generalization in adults, adolescents, children and infants from 1 month of age with epilepsy.
  • Myoclonic seizures in adults and adolescents from 12 years of age with Juvenile Myoclonic Epilepsy.
  • Primary generalized tonic-clonic seizures in adults and adolescents from 12 years of age with Idiopathic Generalized Epilepsy.

Dosage and Administration: Epicet XR tablets should be swallowed whole with liquid and may be taken with or without food. The dose must be individualized according to the patient's condition and response.

• Monotherapy for adults and adolescents from 16 years of age: The recommended starting dose is 250 mg twice daily, which should be increased to an initial therapeutic dose of 500 mg twice daily after two weeks. The dose can be further increased by 250 mg twice daily every two weeks depending upon the clinical response. The maximum dose is 1500 mg twice daily.
• Adjunctive therapy for adults (≥18 years) and adolescents (12 to 17 years) weighing 50 kg or more: The initial therapeutic dose is 500 mg twice daily. This dose can be started on the first day of treatment. Depending upon the clinical response and tolerability, the daily dose can be increased up to 1,500 mg twice daily. Dose changes can be made in 500 mg twice daily increases or decreases every two to four weeks.
• Adjunctive therapy for children 6 years and above and adolescents weighing less than 50 kg: The starting dose is 10 mg/kg body weight twice daily. The dose can be increased every two weeks by 10 mg/kg twice daily to the maximum recommended daily dose of 30 mg/kg twice daily.

In patients with renal impairment, the dose should be adjusted based on creatinine clearance.

Side Effects: Common side effects may include somnolence, asthenia, headache, and infection. Other possible side effects include dizziness, fatigue, nervousness, irritability, aggression, decreased appetite, nausea, and vomiting.

Precautions: Use with caution in patients with pre-existing renal impairment; dosage adjustment may be necessary. Levetiracetam may cause central nervous system depression; caution patients about activities requiring mental alertness. There have been rare reports of suicidal ideation and behavior in patients treated with antiepileptic drugs, including levetiracetam. Monitor patients for signs of depression and suicidal thoughts. If psychotic symptoms occur, discontinuation of treatment should be considered. In pediatric patients, the tablet formulation is not adapted for use in infants and children under the age of 6 years.

Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
