Epnil 1 Tablet contains Clonazepam 1 mg. It is manufactured by Sandoz (A Novartis Division) in Bangladesh.

Indications: Epnil is indicated for the treatment of:

  • Panic disorder, with or without agoraphobia.
  • Lennox-Gastaut Syndrome (petit mal variant), akinetic, and myoclonic seizures, alone or as an adjunct.
  • Absence seizures (petit mal) in patients who have failed to respond to succinimides.

Dosage and Administration: Dosage should be individualized by the physician. General guidelines:

  • Adults with Seizure Disorders: Initial dose should not exceed 1.5 mg/day divided into three doses. Dosage may be increased by 0.5 to 1 mg every 3 days until seizures are controlled or side effects preclude further increase. Maximum recommended daily dose is 20 mg.
  • Adults with Panic Disorder: Initial dose is 0.25 mg twice daily. The target dose of 1 mg/day for most patients may be reached after 3 days. The maximum recommended daily dose is 4 mg.
  • Pediatric Patients: Initial dose should be between 0.01 and 0.03 mg/kg/day (up to 10 years or 30 kg of body weight), not to exceed 0.05 mg/kg/day given in two or three divided doses.

Contraindications: Epnil is contraindicated in patients with:

  • Hypersensitivity to benzodiazepines.
  • Significant liver disease.
  • Acute narrow-angle glaucoma.

Precautions:

  • Use with caution in patients with chronic pulmonary insufficiency or impaired renal function. Dosage reduction may be needed.
  • Elderly or debilitated patients may require lower doses.
  • Use with caution in patients with spinal or cerebellar ataxia or acute intoxication with alcohol or drugs.
  • Avoid abrupt discontinuation of treatment.
  • Monitor patients for suicidal ideation and behavior.
  • Concomitant use with alcohol or other CNS depressants should be avoided due to the potential for increased sedation and respiratory/cardiovascular depression.
  • Use with extreme caution in patients with a history of alcohol or drug abuse.
  • Concomitant use with opioids may result in significant sedation, respiratory depression, coma, and death. Reserve for when alternative options are inadequate and use the lowest effective dose for the shortest duration.
  • In infants and young children, Epnil may increase saliva and bronchial secretions, requiring attention to maintain clear airways.
  • May increase the incidence of generalized tonic-clonic seizures in patients with multiple seizure types.

Side Effects: Common side effects may include:

  • Drowsiness (very common)
  • Ataxia (uncoordinated movements)
  • Dizziness
  • Depression
  • Fatigue
  • Impaired coordination
  • Slurred speech
  • Memory problems

Less common but potentially serious side effects can occur. Report any unusual or severe symptoms to your doctor immediately.

Drug Interactions: Epnil may have additive depressant effects when used with other CNS depressants such as:

  • Alcohol
  • Tricyclic antidepressants
  • MAO inhibitors
  • Sedatives and hypnotics
  • Barbiturates
  • Anxiolytics
  • Antipsychotics The effect of Epnil on the metabolism of other drugs has not been extensively investigated, but it does not appear to alter the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital. However, caution is advised when used with valproic acid as it may increase the possibility of absence status.

Storage: Keep in a dry place away from light and heat. Keep out of the reach of children. Store below 30°C. Do not prepare Epnil infusions using sodium bicarbonate solution, as precipitation may occur.
