Indications
Esocon MUPS (Esomeprazole) is a proton pump inhibitor (PPI) indicated for:
- Gastro-esophageal Reflux Disease (GERD): Including erosive esophagitis and maintenance of healing.
- Risk reduction in NSAID-associated gastric ulcers.
- H. pylori eradication: Used as part of a triple therapy regimen.
- Zollinger-Ellison Syndrome and idiopathic hypersecretion.
Composition & Description
MUPS stands for Multiple-Unit Pellet System. This tablet formulation is made by compacting pellets, combining the benefits of both tablets and pellet-filled capsules. MUPS tablets ensure greater bioavailability, uniform drug absorption, and a lower risk of localized irritation or dose dumping compared to conventional capsules.
Pharmacology
Esomeprazole, a proton pump inhibitor, works by blocking the hydrogen-potassium ATPase enzyme system (the proton pump) in the gastric parietal cells. This action effectively suppresses gastric acid secretion and provides short-term treatment for gastric and duodenal ulcers.
Dosage & Administration
- Erosive Esophagitis (Adults & Adolescents ≥12 years): 40 mg once daily for 4 weeks.
- Maintenance of Healing (Adults & Adolescents ≥12 years): 20 mg once daily.
- NSAID-associated Gastric Ulcer Risk Reduction: 20 mg once daily for 4-8 weeks.
- H. pylori Eradication (Adults & Adolescents ≥12 years): 20 mg twice daily for 7 days, in combination with amoxicillin and clarithromycin.
- Zollinger-Ellison Syndrome: 40-80 mg twice daily.
- Children 1-11 years: Doses range from 10 mg to 20 mg once daily, depending on weight.
- Administration: Tablets should be swallowed whole with water and not chewed or crushed. They can also be dispersed in half a glass of non-carbonated water and consumed immediately.
Interaction
- CYP2C19 Substrates (e.g., Diazepam): Esomeprazole may decrease the clearance of these drugs, leading to increased plasma levels.
- Gastric pH-Dependent Drugs (e.g., Ketoconazole, Iron salts, Digoxin): Esomeprazole may interfere with their absorption.
- Combination Therapy: Co-administration with clarithromycin and amoxicillin increases plasma levels of esomeprazole.
Contraindications
Esomeprazole is contraindicated in patients with a known hypersensitivity to any of its components.
Side Effects
Common side effects include headache, diarrhea, nausea, flatulence, abdominal pain, and dry mouth.
Pregnancy & Lactation
The manufacturer advises caution. It is not known if Esomeprazole or its metabolites are present in human breast milk.
Precautions & Warnings
- Use with caution in patients with severe liver or renal impairment.
- Long-term use (over a year) of a PPI may slightly increase the risk of hip, wrist, and spine fractures.
Storage Conditions
Store in a cool and dry place below 25°C, protected from light. Keep out of reach of children.
