Topira 50 Tablet contains 50 mg of Topiramate. 
It is manufactured by UniMed UniHealth Pharmaceuticals Ltd.  

Indications:

Etopira is indicated for:

  • Epilepsy:
    • As monotherapy in adults and children aged 6 years and above with newly diagnosed epilepsy who have generalized tonic-clonic seizures or partial seizures with or without secondarily generalized seizures. 
    • As adjunctive therapy in adults and children over 2 years of age who are inadequately controlled on conventional first-line antiepileptic drugs for partial seizures with or without secondarilygeneralized seizures; seizures associated with Lennox-Gastaut Syndrome and primary generalized tonic-clonic seizures.  
  • Migraine: Prophylaxis of migraine headache in adults.

Dosage and Administration:

The dosage varies depending on the condition and age of the patient and should be determined by a physician. Some general guidelines:

  • Epilepsy (Monotherapy in Adults and Children over 16 years): Initial dose is usually 25 mg nightly for 1 week, gradually increased. Recommended initial target dose is 100 mg/day, maximum 400 mg/day in two divided doses.
  • Epilepsy (Monotherapy in Children 6-16 years):Initial dose is 0.5 to 1 mg/kg nightly for the first week, gradually increased. Recommended initial target dose range is 3 to 6 mg/kg/day in two divided doses. 
  • Epilepsy (Adjunctive Therapy in Adults and Children over 2 years): The minimal effective dose is 200 mg per day. Usual total daily dose is 200 to 400 mg in two divided doses, with a maximum recommended dose of 800 mg/day. In children (2-16 years), the recommended total daily dose is approximately 5 to 9 mg/kg/day in two divided doses.  
  • Migraine Prophylaxis in Adults: Initial dose is 25 mg nightly for 1 week, increased in 25 mg/day increments at weekly intervals. Recommended total daily dose is 100 mg/day in two divided doses, although some patients may benefit from 50 mg/day.

Etopira tablets can be taken with or without food. It is important to follow your doctor's instructions carefully. 

Side Effects:

Common side effects may include:

  • Nausea
  • Taste change

  • Diarrhea  
  • Weight loss  
  • Loss of appetite  
  • Memory impairment
  • Dizziness  
  • Nervousness
  • Numbness  
  • Psychomotor impairment  
  • Sleepiness  
  • Speech disorder  
  • Upper respiratory tract infection  
  • Visual impairment  
  • Abdominal pain  

Less common but potentially serious side effects can occur. It is important to report any unusual symptoms to your doctor.

Precautions:

  • Inform your doctor about any pre-existing medical conditions, especially kidney or liver disease, history of metabolic acidosis, or eye problems (like glaucoma).
  • Etopira may cause dizziness and sleepiness; use caution when driving or operating machinery.  
  • Stay hydrated by drinking plenty of fluids.
  • Monitor your weight during treatment.
  • Do not stop taking the medication suddenly without talking to your doctor, as it may increase the risk of seizures.  
  • Inform your doctor if you experience any vision problems.
  • Be aware of potential mood changes or suicidal thoughts.

Contraindications:

Etopira is contraindicated in patients with hypersensitivity to any component of the preparation.  

Drug Interactions:

Etopira can interact with various medications. Inform your doctor about all the medicines you are currently taking, including other antiepileptic drugs, oral contraceptives, lithium, hydrochlorothiazide, and metformin. 

Storage:

Store below 30°C, protected from light and moisture. Keep out of reach of children.
