Exformin ER 500

Presentation: Extended-release tablets containing 500 mg of metformin hydrochloride.

Indications: Exformin ER 500 is an oral antidiabetic drug belonging to the biguanide class, formulated for extended release. It is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. Itcan be used as monotherapy or in combination with other antidiabeticagents such as sulfonylureas, thiazolidinediones, DPP-4 inhibitors, SGLT2 inhibitors, or insulin. The extended-release formulation allows for once-daily dosing, which may improve patient adherence.

Dosage and Administration: The usual starting dose of metformin extended-release is 500 mg once daily with the evening meal. The dosage may be gradually increased by your doctor in increments of 500 mg weekly up to a maximum of 2000 mg once daily with the evening meal. If glycemic control is not achievedwith 2000 mg once daily, a dose of 1000 mg twice daily may be considered. Exformin ER 500 tablets should be swallowed whole and should not be crushed, cut, or chewed. Take it with meals to reduce gastrointestinal side effects. Follow your doctor's instructions carefully.

Side Effects: Common side effects of metformin include gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite. These side effects are usually mild and transient, occurring more frequently during the start of treatment and subside over time. The extended-release formulation may help to reduce the incidence and severity of these gastrointestinal effects. Lactic acidosis is a rare but serious metabolic complication that can occur with metformin use, especially in patients with kidney problems. Symptoms include nausea, vomiting, abdominal pain, muscle cramps, severe tiredness, and difficulty breathing. If you experience any of these symptoms, seek immediate medical attention.

Precautions:

  • Inform your doctor about any pre-existing medical conditions, especially kidney disease, liver disease, heart failure, or any serious acute medical illness that can affect kidney function (such as dehydration, severe infection, or shock).
  • Metformin is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min).
  • Temporary discontinuation of metformin may be necessary before surgical procedures or radiological studies involving iodinated contrast agents, as these can potentially worsen kidney function.
  • Excessive alcohol intake should be avoided while taking Exformin ER 500, as it can increase the risk of lactic acidosis.
  • Inform your doctor if you are pregnant or breastfeeding, as metformin use during these periods may require careful consideration.
  • Be aware of potential drug interactions and inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.  

Storage:   Store below 30°C, protected from light and moisture. Keep out of the reach of children.
