Indications

Fenocap (Fenofibrate) is a fibric acid derivative indicated for the treatment of hyperlipidemias of types IIa, IIb, III, IV, and V. It is used in patients who have not responded adequately to dietary changes and other measures.


 

Pharmacology

Fenofibrate is rapidly converted to its active form, fenofibric acid. This acid works by reducing total cholesterol, LDL cholesterol, and triglycerides, while increasing levels of HDL cholesterol. It also helps to lower serum uric acid levels. The micronized formulation of Fenofibrate has improved absorption.


 

Dosage & Administration

  • Hypercholesterolemia or mixed hyperlipidemia: The initial dose for adults is 200 mg per day.
  • Hypertriglyceridemia: The initial dose is 67 to 200 mg per day. Doses should be individualized based on the patient's response and should be taken with a meal to optimize bioavailability.

 

Interaction

  • Warfarin: Fenocap can potentiate the anticoagulant effects of warfarin, increasing the risk of bleeding.
  • Antidiabetic drugs: It may improve glucose tolerance and have an additive effect.
  • Cyclosporine: May increase the nephrotoxicity of cyclosporine.
  • HMG-CoA reductase inhibitors (statins): Use with caution due to the potential risk of rhabdomyolysis, although low-dose statins and Fenocap may be well-tolerated together.

 

Contraindications

Fenofibrate is contraindicated in patients with hypersensitivity to Fenofibrate, severe renal or hepatic impairment, existing gall bladder disease, photosensitivity to ketoprofen, and in breastfeeding mothers.


 

Side Effects

Common side effects include gastrointestinal issues (nausea, anorexia, gastric pain), pruritus, urticaria, impotence, headache, dizziness, vertigo, fatigue, and hair loss. Myotoxicity is a potential side effect.


 

Pregnancy & Lactation

Fenofibrate is not recommended for use in pregnant women or breastfeeding mothers.


 

Precautions & Warnings

 

  • Special care is needed for patients with renal disease. Progressive increases in serum creatinine may lead to myotoxicity.
  • Discontinue the drug if myotoxicity is suspected or if creatinine kinase levels increase significantly.
  • Liver function tests are recommended every 3 months during the first year of treatment.

 

Overdose Effects

There is no specific treatment for an overdose. Supportive care is recommended, including monitoring vital signs. Hemodialysis is not effective because the drug is highly bound to plasma proteins.
