Farobest (Faropenem): A Penem Antibiotic for Bacterial Infections

Farobest contains Faropenem, an antimicrobial agent belonging to the penem class. It possesses bactericidal activity against a wide range of Gram-negative, Gram-positive, and anaerobic bacteria. It is indicated for the treatment of various bacterial infections across different body systems.


How Farobest Works (Pharmacology)

Faropenem works by inhibiting bacterial cell wall synthesis. Its mechanism of action involves:

  • Bactericidal Activity: It kills bacteria.
  • High Affinity for PBPs: Faropenem has a strong affinity for the high molecular penicillin-binding proteins (PBPs) of the bacterial cell wall, particularly PBPs 2 and 1A of Escherichia coli. These proteins are essential for the multiplication of bacilli.
  • Broad Antibacterial Spectrum: It exhibits broad activity against both aerobic and anaerobic Gram-positive and Gram-negative bacteria.
  • Beta-Lactamase Stability: Faropenem is highly stable against various beta-lactamases, which are enzymes produced by some bacteria that can inactivate many other antibiotics.

Farobest Tablets are indicated for the treatment of infections caused by susceptible organisms in the following areas:

  • Lower Respiratory Tract Infections: Acute bronchitis, pneumonia, pulmonary suppuration.
  • Ear, Nose, and Throat (ENT) Infections: Otitis externa (outer ear infection), tympanitis (inflammation of the eardrum), sinusitis.
  • Genito-urinary Infections: Pyelonephritis (kidney infection), cystitis (bladder infection), prostatitis, seminal gland inflammation.
  • Upper Respiratory Tract Infections: Pharyngitis (sore throat), tonsillitis.
  • Skin and Skin Structure Infections: Pustular acne, folliculitis (hair follicle inflammation), contagious impetigo, erysipelas, lymphangitis (inflammation of lymphatic vessels), suppurative nail inflammation, subcutaneous abscess, hidradenitis (sweat gland inflammation), infective sebaceous cyst, chronic pyoderma, secondary infection of external wounds or surgical wounds.
  • Gynecological Infections: Adnexitis (inflammation of uterine adnexa), Bartholin gland inflammation.

Dosage & Administration

The dosage of Farobest varies depending on the type of infection:

  • Upper Respiratory Tract Infections: 150 mg three times a day (t.i.d.), can be increased to 200 mg t.i.d.
  • ENT Infections: 200 mg t.i.d., can be increased to 300 mg t.i.d.
  • Lower Respiratory Tract Infections: 200 mg t.i.d., can be increased to 300 mg t.i.d.
  • Skin and Skin Structure Infections: 150 mg t.i.d., can be increased to 200 mg t.i.d.
  • Genito-urinary Infections: 200 mg t.i.d., can be increased to 300 mg t.i.d.
  • Gynecological Infections: 150 mg t.i.d., can be increased to 200 mg t.i.d.

Important Considerations & Warnings

Contraindications:

  • Patients with known hypersensitivity to any component of this product.
  • Patients with known hypersensitivity to other drugs in the same class (penems).
  • Patients who have demonstrated anaphylactic reactions to beta-lactam antibiotics (e.g., penicillins, cephalosporins).

Side Effects:

  • Common: Headache, Nausea, Vomiting, Stomach pain, and Diarrhea.

Pregnancy & Lactation:

  • Pregnancy: Safety during pregnancy has not been established. Should be given to pregnant women or expectant mothers only if the benefit of the treatment is greater than the risks involved.
  • Lactation: Faropenem is excreted in human milk. Therefore, it should be given to nursing mothers only if the benefits outweigh the risks.

Precautions & Warnings:

Farobest should be administered with caution in the following patient groups:

  • Patients with a past history of hypersensitivity to penicillin, cephem (cephalosporin), or carbapenem drugs.
  • Patients with a family history of atopy (a genetic predisposition to allergic diseases).
  • Patients with renal impairment: The dosage should be reduced or the interval between doses should be increased.
  • Geriatric patients.
  • Patients with poor oral intake or poor general state (due to risk of vitamin K deficiency; proper monitoring should be done).

Drug Interactions:

  • Imipenem and Cilastatin sodium combination: May increase Farobest blood concentrations due to obstruction of metabolic fermentation by cilastatin.
  • Furosemide: Animal studies (dog) suggest an increased kidney toxicity of Farobest when co-administered.
  • Sodium valproate: Concurrent use with carbapenem drugs (like meropenem, panipenem, and imipenem-cilastatin sodium) has been reported to reduce valproic acid concentration in the blood, potentially leading to a recurrence of epileptic fits.

Use in Special Populations:

  • Renal Impairment: Plasma concentrations of the drug are increased, and the half-life is extended. Dose adjustment is necessary.
  • Geriatric Use: The half-life is prolonged in the elderly, possibly due to declining kidney function, leading to higher plasma concentrations. Start with a dose of 150 mg and monitor for undesirable effects. If diarrhea or loose bowel movements occur, stop the medicine, monitor, and take appropriate measures. There is a tendency for hemorrhage due to vitamin K deficiency in the elderly.

Overdose Effects

  • No specific information is available on the treatment of overdosage with Farobest.
  • Intentional overdosing is unlikely.
  • In the event of an overdose, Farobest should be discontinued, and general supportive treatment should be given until renal elimination takes place.
