Folinic 5 

Presentation: Tablets containing Calcium Folinate equivalent to 5 mg of Folinic Acid. Calcium Folinate is a derivative of folic acid. The tablets are intended for oral administration. The appearance may vary depending on the manufacturer and what is available in Bangladesh.

Indications: Folinic 5 is indicated for:

  • Rescue therapy after high-dose methotrexate therapy in osteosarcoma.
  • Treatment of megaloblastic anemias due to folic acid deficiency when oral folic acid administration is unsuitable.
  • In combination with 5-fluorouracil in the treatment of advanced colorectal cancer.
  • Overdosage of folic acid antagonists such as methotrexate, trimethoprim, and pyrimethamine.

Dosage and Administration: The dosage and duration of treatment with Folinic 5 depend greatly on the indication and the specific treatment protocol. It is crucial to follow the prescribing physician's instructions precisely.

  • Methotrexate Rescue: The dosage regimen is highly dependent on the methotrexate dose and the patient's renal function. A typical regimen involves administering Calcium Folinate 15 mg orally, intramuscularly, or intravenously every 6 hours for 6 to 10 doses, starting 24 hours after the beginning of the methotrexate infusion. Dosage adjustments are made based on monitoring of methotrexate levels and creatinine.
  • Megaloblastic Anemia: 5-15 mg daily.
  • Colorectal Cancer: The dosage is determined by the specific chemotherapy regimen used in combination with 5-fluorouracil.
  • Folic Acid Antagonist Overdosage: Dosage depends on the antagonist and the time since its administration.

The tablets are usually administered orally and should be swallowed whole with water.

Side Effects: Side effects of Calcium Folinate are generally mild and infrequent. They may include allergic reactions (skin rash, itching, hives), and gastrointestinal disturbances. When used in combination with chemotherapy, the side effects are often attributable to the other cytotoxic drugs.

Precautions: Calcium Folinate should only be used for the indications listed and under the supervision of a physician experienced in the use of this medication, especially in the context of methotrexate rescue or cancer chemotherapy. It should not be administered intrathecally. Calcium Folinate may increase the frequency of seizures in susceptible individuals. It may also interact with certain antiepileptic drugs, potentially decreasing their efficacy.
