Presentation: Extended-release tablets containing metformin hydrochloride.
Indications: Management of type 2 diabetes mellitus in adults, especially in overweight patients, when diet and exercise alone are insufficient for adequate glycemic control. May be used as monotherapy or in combination with other oral antidiabetic agents or insulin.
Dosage and Administration: Usually initiated at 500 mg or 750 mg once daily with the evening meal. Dosage may be increased gradually in weekly increments based on glycemic control and tolerability, up to a maximum of 2000 mg once daily with the evening meal (for 500 mg and 750 mg strengths) or 2250 mg once daily (for 750 mg strength) or 2000 mg daily in divided doses (for 1000 mg strength). Swallow tablets whole; do not crush or chew. Individualized based on glycemic control and tolerability. Contraindicated in patients with eGFR below 30 mL/min/1.73 m², and not recommended to initiate in patients with eGFR between 30-45 mL/min/1.73 m². Assess renal function before starting and at least annually thereafter. Take with meals to reduce gastrointestinal side effects.
Side Effects: Common side effects include diarrhea, nausea, vomiting, flatulence, and abdominal discomfort, which may decrease over time. Metallic taste may occur. Long-term use may lead to vitamin B12 deficiency. Lactic acidosis is a rare but serious potential side effect.
Precautions: Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73 m²), metabolic acidosis (including diabetic ketoacidosis), and known hypersensitivity to metformin. Use with caution in patients with moderate renal impairment (eGFR 30-45 mL/min/1.73 m²), liver disease, heart failure, and excessive alcohol intake due to increased risk of lactic acidosis. Temporary discontinuation may be required for surgical procedures or radiologic studies involving iodinated contrast agents.
