💊 Frusemide & Spironolactone (Combined Diuretic)

This medication combines two different types of water pills (diuretics) to help the body remove excess fluid. Frusemide is a powerful "loop diuretic," while Spironolactone is a "potassium-sparing diuretic" that helps balance the potassium loss often caused by Frusemide.

📋 Key Indications

• Edema: Swelling caused by excess fluid retention.

• Heart Health: Chronic congestive heart failure.

• Liver Health: Hepatic cirrhosis with fluid in the abdomen (ascites).

• Hypertension: Essential high blood pressure.

• Hyperaldosteronism: Conditions where the body produces too much aldosterone.

🥄 Dosage & Administration

The dose is adjusted based on patient response and whether they are already stabilized on these medications.

• Low Strength (Frusemide 20 mg / Spironolactone 50 mg): 1 to 4 tablets daily.

• High Strength (Frusemide 40 mg / Spironolactone 50 mg): 1 to 2 tablets daily.

• Pediatric Use: 🛑 This combination is not suitable for children.

• Elderly: Use with caution, as the medication may be processed more slowly by the body.

⚠️ Safety & Interactions

• Critical Warning: Do not use if you have severe kidney impairment (Creatinine clearance <30 ml/min), Addison's disease, or high potassium levels (hyperkalaemia).

• Drug Interactions:

  • ACE Inhibitors: Increased risk of dangerously high potassium.

  • Digoxin: May increase the risk of digitalis toxicity.

  • NSAIDs (like Ibuprofen): May reduce the effectiveness of the diuretic.

  • Aminoglycoside Antibiotics: Frusemide can increase the risk of hearing damage (ototoxicity).

• Side Effects: Common issues include headache, GI distress, and skin rashes. Hormonal changes like breast enlargement in men (gynaecomastia) or menstrual irregularities in women may occur.

🤰 Pregnancy & Lactation

• Pregnancy: Should only be used if the potential benefits outweigh the risks to the fetus.

• Lactation: Both medications are excreted in breast milk. If treatment is essential, breastfeeding should be discontinued and an alternative feeding method used.
