Indications:Galvus-Met is indicated in adults with type 2 diabetes mellitusas an adjunct to diet and exercise to improve glycemic controlin patients whose diabetes is not adequately controlled with metforminor vildagliptin alone, or who are already treated with the combination of vildagliptin and metformin as separate tablets. It can also be used as initial therapy in some patients when diabetes is not adequately controlled by diet and exercise alone. Galvus-Met can be used in combination with a sulfonylurea or with insulin.

Dosage and Administration: The dosage of Galvus-Met should be individualized based on the patient's current regimen, effectiveness, and tolerability, without exceeding the maximum recommended daily dose of vildagliptin 100 mg and metformin 2000 mg. Galvus-Met 50/500 is usually administered twice daily with meals, with one tablet taken in the morning and the other in the evening.

• The starting dose should be based on the patient's current antidiabetic regimen.
• For patients not currently treated with metformin, the recommended starting dose is usually 50 mg vildagliptin/500 mg metformin hydrochloride twice daily.
• For patients switching from metformin monotherapy, the starting dose of Galvus-Met should provide the same total daily dose of metformin as was previously taken, plus vildagliptin 50 mg twice daily.
• For patients switching from separate tablets of vildagliptin and metformin, Galvus-Met should be initiated at the same total daily doses of each component.
• Renal function should be assessed before initiating Galvus-Met and periodically thereafter.

Side Effects: Common side effects may include nausea, diarrhea, headache, and upper respiratory tract infection. Hypoglycemia (low blood sugar) may occur when Galvus-Met is used in combination with insulin or sulfonylureas.

Precautions:Galvus-Met is contraindicated in patients with known hypersensitivity to vildagliptin or metformin hydrochloride or any of the excipients, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or a history of serious hypersensitivity reaction to sitagliptin or metformin.It is not recommended for use in patients with type 1 diabetes mellitus. Caution should be exercised in patients with moderate renal impairment, a history of pancreatitis, heart failure, and excessive alcohol intake due to the risk of lactic acidosis associated with metformin. Liver function tests may be recommended before starting treatment and periodically during treatment.
