Gastalfet (Sucralfate): A Chelating Complex for Ulcer Treatment

Gastalfet contains Sucralfate, a non-systemic drug that is minimally absorbed by the body. It is indicated for the treatment of duodenal ulcers, gastric ulcers, and chronic gastritis. It is also used for the prophylaxis of gastrointestinal hemorrhage from stress ulceration in seriously ill patients. Gastalfet belongs to the therapeutic class of Chelating complexes.


How Gastalfet Works (Pharmacology)

Sucralfate's mechanism of action is to form a protective barrier over the ulcerated area:

  • Binds to the Ulcer Site: It forms a chemical complex that selectively binds to the proteins at the ulcer site.
  • Creates a Protective Barrier: This binding creates a protective barrier that shields the ulcer from further damage by stomach acid, pepsin, and bile.
  • Promotes Healing: By protecting the ulcerated area, it allows the natural healing process to occur.
  • Inhibits Pepsin and Bile: It also directly inhibits the actions of pepsin and bile, which can further irritate and damage ulcers.

Key Indications & Benefits

Gastalfet is indicated for the treatment of:

  • Duodenal ulcer
  • Gastric ulcer
  • Chronic gastritis
  • Prophylaxis of gastrointestinal hemorrhage from stress ulceration in seriously ill patients.

Dosage & Administration

Gastalfet should be taken on an empty stomach. Antacids should not be taken within 30 minutes of Gastalfet. The duration of treatment depends on the condition. Always consult a registered physician for specific dosage instructions.

  • Duodenal/Gastric Ulcer (Adults):
    • 2 gm twice daily (on rising and at bedtime) OR
    • 1 gm four times a day (one hour before meals and at bedtime).
    • Treatment may be extended up to 12 weeks for resistant cases.
  • Prophylaxis for stress ulceration (Adults):
    • 1 gm orally or via a nasogastric tube 4 to 6 times a day.
    • The nasogastric tube should be flushed with water after each dose to prevent clogging.
    • Treatment should not normally exceed 14 days.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking Gastalfet.

Contraindications:

  • Known hypersensitivity to sucralfate.

Side Effects:

  • Most common: Headache (3.4%), nausea, abdominal pain, constipation, and diarrhea.
  • Rare: Bezoar formation (a compacted mass of undigested material) has been reported in severely ill patients, especially those with delayed gastric emptying or on enteral tube feedings.
  • Hyperglycemia: Episodes of hyperglycemia have been reported in diabetic patients.

Pregnancy & Lactation:

  • Pregnancy: Safety has not been established. It should be used during pregnancy only if clearly needed.
  • Lactation: It is unknown if the drug is excreted in human milk, so caution should be exercised.

Precautions & Warnings:

  • Renal Dysfunction: Use with caution in patients with renal dysfunction due to the possibility of increased aluminum absorption.
  • Severe Renal Impairment: Not recommended for individuals on dialysis, and should be used with extreme caution and for short-term treatment only in patients with severe or chronic renal impairment. Aluminum accumulation can lead to serious complications.
  • Bezoars: Caution is needed in severely ill patients, especially with conditions that predispose to bezoar formation.
  • Concomitant Medications: Do not co-administer with citrate preparations as this can increase aluminum absorption.

Drug Interactions:

  • Reduced Bioavailability: Gastalfet can reduce the bioavailability of many drugs, including fluoroquinolones (ciprofloxacin, norfloxacin), tetracycline, digoxin, warfarin, phenytoin, and others.
  • Separated Administration: To avoid interactions, the administration of these other drugs should be separated from Gastalfet by at least two hours.
  • Enteral Feeds: In patients receiving nasogastric tube feeding, Gastalfet should be administered separately from the enteral feed (by one hour) to prevent bezoar formation.

Overdose Effects

  • Overdoses have been largely asymptomatic, with some reports of abdominal pain, nausea, and vomiting.
  • The risk associated with overdose is considered minimal.

Storage Conditions

Store in a cool and dry place, protected from light.
