Gavisol: An Antacid and Alginate for Reflux and Heartburn
Gavisol is a combination of two antacids (Calcium Carbonate and Sodium Bicarbonate) and an alginate (Sodium Alginate). It is indicated for the treatment of gastric reflux, heartburn, indigestion, and flatulence associated with reflux. It is particularly useful for heartburn during pregnancy and other cases of epigastric and retrosternal distress caused by gastric reflux. Gavisol belongs to the therapeutic class of Antacids.
How Gavisol Works (Pharmacology)
Gavisol's mode of action is physical and twofold, without being absorbed into the systemic circulation:
- Neutralizes Stomach Acid: The antacids (Calcium Carbonate and Sodium Bicarbonate) rapidly neutralize excess stomach acid, providing quick relief from pain and discomfort.
- Forms a Protective Raft: The alginate component reacts with gastric acid to form a protective gel or "raft" of Alginic Acid. This raft floats on top of the stomach contents, physically impeding gastro-esophageal reflux for up to four hours.
- Soothes the Esophagus: This raft effectively caps the acid pocket in the stomach, reducing acid exposure to the esophagus. In severe cases, the raft itself may be refluxed, providing a soothing, demulcent effect.
- Traps Bile and Pepsin: In addition, the raft can entrap bile and pepsin, further protecting the esophagus from these gastric components.
Key Indications & Benefits
Gavisol is indicated for:
- Gastric reflux and associated heartburn.
- Indigestion.
- Heartburn of pregnancy.
- Epigastric and retrosternal distress where the cause is gastric reflux.
Dosage & Administration
Gavisol is available as an oral suspension and chewable tablets. Dosage varies with age and should be taken after meals and at bedtime.
- Oral Suspension (Adults and children over 12): 10-20 mL after meals and at bedtime, up to four times a day.
- Oral Suspension (Children 6-12): 5-10 mL after meals and at bedtime, up to four times a day.
- Chewable Tablets (Adults and children 12 and over): Two to four tablets after meals and at bedtime, up to four times a day.
- Children under 6: Not recommended for the suspension. For tablets, it should be given only on medical advice.
- Elderly and Hepatic Impairment: No dose modification is necessary.
Important Considerations & Warnings
It is crucial to discuss your full medical history with your doctor before taking Gavisol.
Contraindications:
- Known or suspected hypersensitivity to the active ingredients or any excipients.
Side Effects:
- Common: Constipation, flatulence, stomach cramps, or belching.
- If a large dose is taken: A sensation of swelling may occur. Consult a physician in this case.
Pregnancy & Lactation:
- Pregnancy: It can be used during pregnancy if clinically needed, as clinical studies and post-marketing data indicate no harm to the fetus.
- Breastfeeding: It can be used during breastfeeding as no effects have been shown in breastfed infants.
- Fertility: Clinical data do not suggest an effect on human fertility.
Precautions & Warnings:
- High Sodium Content: The suspension has a high sodium content (254.5 mg per 20 mL), which should be considered for patients on a highly restricted salt diet (e.g., congestive heart failure, renal impairment). The chewable tablets also contain sodium.
- Calcium Content: It contains calcium (260 mg per 20 mL suspension; 75 mg per tablet). Caution is needed for patients with hypercalcemia, nephrocalcinosis, and a history of calcium-containing kidney stones.
- Phenylketonuria: Chewable tablets contain aspartame and should not be given to patients with phenylketonuria.
- Masking Symptoms: Gavisol may mask the symptoms of more serious underlying medical conditions. If symptoms persist for more than seven days, seek medical advice.
- Renal Insufficiency: Use with caution if a highly restricted salt diet is necessary. Do not use in patients with impaired renal function/insufficiency and hypophosphatemia.
- Children: Treatment of children under 12 is generally not recommended, except on medical advice.
Drug Interactions:
- A two-hour time interval should be left between taking Gavisol and other medications, especially Tetracyclines, Digoxine, Fluoroquinolone, Iron salts, Ketoconazole, Thyroid Hormones, and others. This is because Gavisol can interfere with the absorption of these drugs.
Overdose Effects
- Overdose may cause abdominal distension.
- Symptomatic treatment should be provided.
Storage Conditions
Store in a cool (below 30°C) and dry place, away from light. Do not refrigerate or freeze. Keep out of the reach of children.
