Glims 4 

Presentation: Glims 4 Tablet, available in Bangladesh, contains Glimepiride 4 mg.

Indications: Glims is indicated for the treatment of type 2 diabetes mellitus in adults when diet, exercise, and weight reduction alone are not adequate to achieve glycemic control. It can be used as monotherapy or in combination with other oral antidiabetic agents or insulin.

Dosage and Administration: The dosage of Glims must be individualized based on the patient's blood glucose levels. The usual starting dose is 1 mg once daily, taken shortly before or with the first main meal of the day (usually breakfast).

• Dosage adjustments are made gradually based on blood glucose response. The dose may be increased in increments of 1 mg at intervals of 1-2 weeks.
• The usual maintenance dose ranges from 1 to 4 mg once daily. In some cases, higher doses up to a maximum of 8 mg once daily may be required.
• If a dose is missed, it should not be taken later in the day. The next dose should be taken at the usual time the following day.
• It is important to have regular meals after taking this medication to minimize the risk of hypoglycemia.

Side Effects: The most common side effect is hypoglycemia (low blood sugar). Symptoms can include headache, dizziness, sweating, weakness, increased hunger, tremor, anxiety, confusion, palpitations, and in severe cases, loss of consciousness or seizures. Gastrointestinal disturbances such as nausea, vomiting, and abdominal discomfort may occur, although they are usually mild and transient. Less common side effects include allergic skin reactions (rash, itching, urticaria), blurred vision, and changes in liver function tests (e.g., elevated transaminases, cholestasis, jaundice). In rare cases, blood disorders such as thrombocytopenia and leukopenia may occur.

Precautions: Glims is contraindicated in patients with:

• Known hypersensitivity to glimepiride or other sulfonylureas or sulfonamides.
• Type 1 diabetes mellitus.
• Diabetic ketoacidosis, diabetic pre-coma and coma.
• Severe renal or hepatic impairment.
• Treatment with bosentan.

It is generally not recommended during pregnancy and breastfeeding. Caution should be exercised in patients with moderate renal or hepatic impairment, elderly or debilitated patients, and in conditions where blood glucose control may be unstable (e.g., intercurrent illness, surgery, stress). Alcohol consumption should be avoided as it can increase the risk of hypoglycemia. Certain medications can interact with glimepiride, potentially affecting blood glucose control; therefore, it is important to inform the doctor about all other medications being taken. Regular monitoring of blood glucose levels is essential.

Storage: Store Glims 4 tablets in a cool (below 30°C) and dry place, protected from light and moisture. Keep this medicine out of the reach of children. Do not use after the expiry date stated on the packaging.
