Presentation: Each extended-release tablet contains Metformin Hydrochloride BP 500 mg. Available in strips.
Indications: Management of type 2 diabetes mellitus, particularly in overweight patients when diet and exercise alone are insufficient for glycaemic control. May be used as monotherapy or in combination with other oral antidiabetic agents or insulin. The extended-release formulation allows for once-daily dosing, which may improve patient compliance.
Dosage and Administration: Adults: The usual starting dose is 500 mg once daily with the evening meal. The dose may be increased gradually in increments of 500 mg weekly, up to a maximum of 2000 mg once daily with the evening meal. If glycemic control is not achieved with 2000 mg once daily, a dose of 1000 mg twice daily may be considered. Swallow the tablet whole; do not crush or chew. Follow the physician's instructions carefully.
Side Effects: Very common side effects include gastrointestinal disturbances such as nausea, vomiting, diarrhoea, abdominal pain, and loss of appetite, particularly at the start of treatment. These are usually transient. A metallic taste may also occur. Long-term use may lead to Vitamin B12 deficiency. The extended-release formulation may reduce the incidence of gastrointestinal side effects compared to immediate-release metformin.
Precautions: Contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute metabolic acidosis, diabetic ketoacidosis, and conditions that may cause tissue hypoxia (e.g., heart failure, recent myocardial infarction, severe infection). Use with caution in patients with moderate renal impairment, liver disease, and in elderly patients. Avoid excessive alcohol intake. May need to be temporarily discontinued before surgery or radiological procedures involving iodinated contrast media.
Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
