Glycema 10 

Presentation:Glycema 10 Tablet, available in Bangladesh, contains Dapagliflozin Propanediol equivalent to Dapagliflozin 10 mg.

Indications:Glycema is indicated for the treatment of type 2 diabetes mellitusin adults as an adjunct to diet and exercise to improve glycemic control.Itcan be used as monotherapy when metformin is considered inappropriate due to intolerance or contraindications. It is also indicated in combination with other glucose-lowering medicinal products including metformin, sulfonylureas, insulin, and thiazolidinediones.

Additionally, Glycema is indicated in adults with type 2 diabetes mellitus and established cardiovascular disease or multiple cardiovascular risk factors to reduce the risk of:  

• Cardiovascular death 
• Hospitalizationfor heart failure

Dosage and Administration: The recommended starting dose of Glycema is 5 mg once daily, taken in the morning, with or without food. In patients tolerating 5 mg once daily who require additional glycemic control, the dose can be increased to 10 mg once daily.  

• Assessment of renal function is recommended prior to initiation ofGlycema therapy and periodically thereafter.
• Glycema should not be initiated in patients with an estimated glomerular filtration rate (eGFR) less than 60 mL/min/1.73 m².
• If eGFR falls below 60 mL/min/1.73 m² during treatment, the 10 mg dose should be stopped.
• No dose adjustment is required based on hepatic impairment.
• The tablet should be swallowed whole and not crushed, split, or chewed.  

Side Effects: Common side effects may include:

• Genital mycotic infections (fungal infections of the vagina or penis) 
• Urinary tract infections 
• Increased urination (polyuria)

Less common side effects can include:

• Nasopharyngitis 
• Nausea 
• Constipation 
• Thirst 
• Dizziness 
• Rash

Hypoglycemia may occur when Glycema is used in combination with insulin or insulin secretagogues (such as sulfonylureas). Rare but serious side effects include diabetic ketoacidosis.

Precautions: Glycema is contraindicated in patients with:

• Hypersensitivity to dapagliflozin or any of the excipients. 
• Severe renal impairment (eGFR < 30 mL/min/1.73 m²), end-stage renal disease, or patients requiring dialysis.
• Active bladder cancer.

It is not recommended for use in patients with moderate renal impairment (eGFR 30 to < 60 mL/min/1.73 m²). Caution should be exercised in patients with a history of hypotension, urinary tract infections, and cardiovascular disease. Volume status should be assessed and corrected in patients with hypovolemia prior to initiating Glycema. Patients should be informed about the risk of ketoacidosis, a serious condition that requires urgent medical attention, and the associated symptoms (such as nausea, vomiting, abdominal pain, tiredness, and difficulty breathing). Temporary interruption of Glycema may be considered in situations of prolonged fasting, severe infection, or major surgery.

Storage:Store Glycema 10 tablets below 30°C, protected from light and moisture.Keep out of reach of children.
