iMAX (Esomeprazole): A Proton Pump Inhibitor (PPI)

iMAX contains Esomeprazole, a proton pump inhibitor (PPI). It is the S-isomer of omeprazole and provides better acid control. iMAX is indicated for the relief of chronic heartburn and other symptoms of GERD, the healing of erosive esophagitis, and the maintenance of that healing. It is also used in combination with other antibiotics to eradicate Helicobacter pylori infection, and for the treatment of Zollinger-Ellison Syndrome, acid-related dyspepsia, and duodenal and gastric ulcers.

How iMAX Works (Pharmacology)

Esomeprazole's mechanism of action is to specifically inhibit gastric acid production:

• Inhibits the Proton Pump: It suppresses gastric acid secretion by specifically inhibiting the H+/K+-ATPase enzyme system, which is also known as the "proton pump."
• Reduces Gastric Acid: The proton pump is found in the gastric parietal cells and is the final step in acid production. By inhibiting this pump, iMAX effectively reduces the amount of acid in the stomach.

Dosage & Administration

iMAX is available as oral dosage forms (tablets, capsules, and oral suspension) and as an IV injection or infusion. Oral forms should be taken one hour before a meal. The dosage and duration of treatment vary widely depending on the condition being treated. Always consult a registered physician for specific dosage instructions.

• Healing of Erosive Esophagitis (Adults): 20 mg or 40 mg once daily for 4-8 weeks.
• Symptomatic GERD (Adults): 20 mg once daily for 4 weeks.
• H. pylori Eradication: A 10-day triple therapy regimen including iMAX 40 mg once daily, Amoxicillin 1000 mg twice daily, and Clarithromycin 500 mg twice daily.
• Zollinger-Ellison Syndrome: 20-80 mg once daily.
• IV Injection/Infusion (Adults): 20 mg or 40 mg once daily for GERD. Higher doses are used for the reduction of rebleeding from ulcers.
• Pediatric Patients: Doses are weight-based and are used for GERD in patients aged 1 month to 17 years.

Contraindications:

• Known hypersensitivity to any of the components of the formulation.

Side Effects:

• Most frequently occurring: Headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth.
• These side effects are similar in both short-term and long-term treatment.

Pregnancy & Lactation:

• Pregnancy: There are no adequate studies in pregnant women. Animal studies have not shown harmful effects. It should be used during pregnancy only if the potential benefit justifies the potential risk.
• Lactation: It is not known if esomeprazole is excreted in human milk, so breastfeeding should be discontinued if the use of the drug is considered essential.

Precautions & Warnings:

• Gastric Malignancy: A symptomatic response to therapy with iMAX does not rule out the presence of gastric malignancy.
• Swallowing Capsules: Patients who have difficulty swallowing can mix the capsule pellets with applesauce and swallow immediately without chewing. The mixture should not be stored.
• Hepatic Insufficiency: In patients with severe hepatic insufficiency, a dose of 20 mg once daily should not be exceeded. No adjustment is needed for mild to moderate cases.

Drug Interactions:

• CYP2C19: iMAX is a substrate for CYP2C19 and can potentially inhibit it. This may decrease the clearance and increase the plasma levels of drugs that are also substrates of this enzyme, such as diazepam.
• Gastric pH: Since iMAX inhibits gastric acid secretion, it may interfere with the absorption of drugs where gastric pH is important for bioavailability (e.g., ketoconazole, iron salts, and digoxin).
• Combination Therapy: When used with clarithromycin and amoxicillin, plasma levels of iMAX and a metabolite of clarithromycin may increase.

Overdose Effects

• There have been no reports of overdose with iMAX in humans.
• In case of overdose, treatment should be symptomatic and supportive.
• Esomeprazole is extensively protein-bound and is not expected to be removed by dialysis.
