Insimet 850

Presentation: Each film-coated tablet contains 850 mg of metformin hydrochloride.

Indications: Insimet 850 is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight patients when diet and exercise aloneare not sufficient to achieve adequate glycemic control. It can be used as monotherapy or in combination with other oral antidiabetic agents or insulin. In children from 10 years of age and adolescents,it may be used as monotherapy or in combination with insulin.

Dosage and Administration: Insimet 850 tablets are for oral administration and should be swallowed whole with water, preferably with meals to minimize gastrointestinal side effects.

• Adults: The usual starting dose is 500 mg twice a day or 850 mg once a day, given with meals. Dosage increases should be made in increments of 500 mg weekly or 850 mg every 2 weeks, up to a total of 2000 mg per day, given in divided doses. For thoserequiring additional glycemic control, the maximum daily dose can be up to 2550 mg, usually given three times a day with meals.
• Children (from 10 years of age and adolescents):The usual starting dose is 500 mg twice a day, given with meals. Dosage increases should be made in increments of 500 mg weekly up toa maximum of 2000 mg per day, given in divided doses.  
• Renal Impairment: Renal function should be assessed before initiating Insimet 850 and regularly thereafter. Dosage adjustments are based on eGFR levels, with use contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73 m²).

Side Effects:

• Very Common: Gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite.
• Common: Taste disturbance.
• Rare: Lactic acidosis (a serious metabolic complication requiring immediate medical attention), decreased vitamin B12 absorption with long-term use.

Precautions:

Insimet 850 is contraindicated in patients with severe renal impairment, acute metabolic acidosis, and conditions leading to tissue hypoperfusion or hypoxemia. Caution should be exercised in patients with moderate renal impairment, elderly patients, and those with conditions predisposing to lactic acidosis. Alcohol consumption should be avoided. Temporary discontinuation may be necessary for surgical procedures or iodinated contrast imaging.

Storage: Store below 30°C, protected from moisture. Keep out of reach of children.
