Ipec-Plus is a film-coated tablet containing a combination of vital nutrients:
- Iron (III) Hydroxide Polymaltose Complex INN 188 mg (equivalent to 47 mg elemental Iron)
- Folic Acid BP 0.5 mg
- Zinc Sulfate Monohydrate USP 61.8 mg (equivalent to 22.5 mg elemental Zinc)
It is manufactured by Aristopharma Ltd. in Bangladesh.
Indications:
Ipec-Plus is a dietary supplement primarily used for the prevention and treatment of iron, folic acid, and zinc deficiencies, especially during:
- Pregnancy
- Lactation (breastfeeding)
- Other conditions requiring these nutrient supplements.
Pharmacology:
- Iron (III) Hydroxide Polymaltose Complex: This is a stable, non-ionic form of iron that allows for controlled absorption when it comes into contact with the mucosal cell surface. This non-ionic nature reduces the likelihood of interactions with food components (like phytates, oxalates, tannins) or other medications (like tetracyclines and antacids) that often interfere with conventional iron absorption. Iron is essential for hemoglobin formation and oxygen transport.
- Folic Acid: Crucial for nucleoprotein synthesis and the formation of red blood cells. It is especially important for proper fetal development and preventing neural tube defects during pregnancy.
- Zinc: A vital cofactor for over 100 enzymes, playing a role in DNA synthesis, protein metabolism, immune function, and maintaining senses of smell and taste.
Dosage and Administration:
Ipec-Plus is taken orally. For maximum absorption, it is generally recommended to take it on an empty stomach, but it can be taken with or after meals to minimize gastrointestinal irritation.
- Recommended Dose:One tablet daily.
- In severe deficiencies: Up to two tablets daily may be required, as advised by a physician.
- Do not exceed the recommended dose without consulting a doctor.
Side Effects:
Ipec-Plus is generally well tolerated. Occasionally, gastrointestinal irritations may occur, which are typically mild and transient. These can include:
- Sensation of repletion (fullness)
- Pressure in the epigastric region (upper abdomen)
- Nausea
- Constipation
- Diarrhea
- Temporary darkening of stool (this is a harmless and common side effect of iron supplementation).
- Rarely, a few allergic reactions may be seen.
Contraindications:
Ipec-Plus is contraindicated in individuals with:
- Known hypersensitivity (allergy) to any of the ingredients in the formulation (Iron Polymaltose Complex, Folic Acid, or Zinc Sulfate).
Precautions and Warnings:
- Iron Overload: Care should be taken in patients who may develop iron overload, such as those with hemochromatosis (a genetic disorder causing too much iron to build up in the body), hemolytic anemia, or red cell aplasia.
- Stool Discoloration: Dark discoloration of the stool is a harmless side effect of iron therapy and is without clinical importance.
- Accidental Overdose: Keep out of children's reach, as accidental overdose of iron-containing products is a leading cause of fatal poisoning in children under 6 years.
Pregnancy and Lactation:
- Pregnancy: While essential during pregnancy for many, iron supplementation should only be initiated in the first trimester if iron deficiency is clearly diagnosed. It is generally considered safe and recommended for use during the second and third trimesters of pregnancy, and during lactation, when prescribed by a doctor to prevent and treat deficiencies.
- Lactation: Safe for use during lactation when prescribed, as these nutrients are important for both mother and infant.
Drug Interactions:
Due to the non-ionic nature of the iron polymaltose complex in Ipec-Plus, ionic interactions with food components (like phytates, oxalates, tannins) and concomitant administration of certain medications (like tetracyclines and antacids) are unlikely to occur, which is a benefit over conventional iron forms.
- Folic Acid interactions:
- Antiepileptics, oral contraceptives, anti-TB drugs, alcohol, aminopterin, methotrexate, pyrimethamine, trimethoprim, and sulfonamides may result in a decrease in serum folate concentrations.
- Folic acid may decrease serum phenytoin concentrations.
Storage Conditions:
Store in a cool and dry place, away from direct light and moisture. Keep the container tightly closed and out of reach of children.
